School Fees Waiver
All students in FCPS are automatically receiving free breakfast and lunch. However, if you would like to request that school fees be reduced/waived for your student, you will need to apply for assistance.

Health Information
Form for students
The Health Information Form provides valuable information regarding student health conditions. These forms are critical in the identification of students who may need healthcare support in the school environment. This form is required for ALL students and was included in our Back to School information on our website. If you have already submitted the form, no further action is needed.
If you have not done so already, please email a completed Health Information Form to LMSHealthForms@fcps.edu or send a hard copy in with your student to the main office.
Digital Consent Form
Families, We Need Your Consent for Select Digital Resources
At Liberty our teachers will use a variety of digital resources to support student learning and promote communication and collaboration with students and families. Some of the digital resources your child may use this year will require your consent before they can be used.
You can find our school’s list of the resources that require parental consent here: https://libertyms.fcps.edu/adopteddigitalresources. Additionally, you can find more information about approved digital resources in FCPS here: https://www.fcps.edu/resources/technology/digital-resources.
Included on this page is also a link to a form where the enrolling parent or guardian will need to indicate whether or not their student is allowed to use the list of digital resources this year. This page also provides more information about how FCPS carefully reviews digital resources for approval and why we must request your consent before using certain tools. We encourage you to complete the parental consent for digital resources form as soon as possible, as your child will not be able to use the digital resources until we have received your permission. Please reach out to Shane Zavala, our School Based Technology Specialist at sazavala@fcps.edu if you have questions or concerns about the digital resources we’ve chosen to use this year.
Bus Notes and Attendance Reminders
Bus Notes:
If your student is interested in riding the bus home with a friend, no problem! We need a parent note from each parent, one for the student riding the alternate bus and one from the parent approving another student to come to their house after school. Each note needs to be brought to the main office at the beginning of the school day and a bus pass will be available for the student riding the alternate bus at the end of the school day. Thank you!
Attendance Reminders:
If your student is going to miss part or a full day of school, please reach out to the attendance line in order to get their excused absence recorded. The phone number is 7039888181, the email is LibertyMiddle.Attendance@fcps.edu, or you can submit an online form from our website main page.
If your student will be picked up early, please send them with a note to the main office with their name, the time they will be picked up and the name of the person who will be picking them up. Students bring the note to the main office at the beginning of the school day and an early dismissal pass will be giving to avoid having to call them out of class. They will be able to meet you here in the office and cut down the time waiting. Thank you!

Thank you to those that were able to join us for our first meeting of the school year. Congratulations to our NEW Board Members, Christine Heath, Jeanne Laszewski, Cindy Mazur and Madison Flemming!
The PTA is here to support parents, teachers, kids, the ENTIRE school community. We can't do it without you. By joining the PTA you help us fund school enrichment programs, teacher grants, school community events, and so much more. Your membership does not commit you to volunteering, it simply shows your support of Liberty Middle School.
