VHSL Physical Form *NEW REVISION* for 2021-2022

The VHSL has released their latest version of the VHSL Physical Form, which was revised in January 2021. Due to the extensive amount of changes in this document, FCPS (Lake Braddock) will ONLY accept this new version of the VHSL physical form for athletic participation in 2021-2022. Physicals submitted for the 2020-2021 school year expired on June 30^th and are no longer valid. Any VHSL physical exam submitted for the 2021-2022 school year must be dated on or after May 1, 2021 and be completed on the Revised January 2021 version. ELECTRONIC SUBMISSION OF VHSL PHYSICAL FORM

• A single PDF document including all four pages of the VHSL form and any other related documents (doctor's notes, specialist clearance, Kaiser Permanente printouts) can be scanned and emailed to LBSSActivities@fcps.edu.
• If you do not have access to a scanner, you can use your phone!
• iPhone Users: There is a built-in scanning feature in the 'Notes' app. Just start a new note, tap on the camera icon, and select 'Scan Documents'. Scan a page, select 'Keep Scan', and continue this process until you have scanned all necessary pages. Click 'Save' in the bottom right corner when finished. You can then tap on the document and email it!
• Android Users: There is a built-in scanning feature in the 'Google Drive' app. Open your Google Drive and tap on the '+' in the bottom right corner. Select the 'Scan' option, which should open your camera. Take a picture of the first page, click 'Done' and then make any adjustments. From here you will tap on the '+' icon in the bottom left corner to scan additional pages into the same document. Continue this process until you have scanned all necessary pages. Click 'Save' in the bottom right corner and then rename the document whatever you would like. You can then open the document and email it!
• Pictures will NO LONGER be accepted for valid submission. Pictures are typically dark or are poor quality by the time they travel through the FCPS email server, making it difficult to read them clearly. Please DO NOT send pictures of this document.

Athletics & Activities Registration for 2021-2022

We have cleared out the 2020-21 activities and opened registration for the 2021-22 school year! Rising 9^th graders have been uploaded into the system as well, allowing parents to register their child for the upcoming sports seasons! This new system collects the Emergency Care information and Concussion Education requirement electronically, requiring less paperwork and simplifying the tryout process. Please visit the Lake Braddock Activities & Athletics Registration Website to get your child set up for each sport they intend to tryout for next year! On the homepage, you will find step-by-step instructions for returning users and new users. There are also some helpful FAQs and a Registration Guide available for reference!

Tryout Card Distribution for 2021-2022

You must have a valid VHSL physical and Athletic Registration complete prior to picking up a Tryout Card. Don’t forget to get these done before Tryout Card Distribution! Please visit the Lake Braddock Athletics Website for more details and coaches contact information.
