Forms

  • Health Information Form and Emergency Care Form – Required Update each school year
    • These are required and provide us with necessary health and emergency contact information for your child. The forms can be filled out via weCare@School, which can found in SIS ParentVUE. Instructions on how to fill them out can be found at the link above. If you need a hard copy of either form please contact Leslie Dorsey at ladorsey@fcps.edu

Optional Forms

Here are some optional forms that parents/guardians can choose to fill out at the beginning of the school year. Any complete optional forms should be return to Kevin McMahon (kjmcmahon@fcps.edu) in Student Services.

  • FLE Opt Out Forms
    • These are optional if you want your child to opt out of Family Life Education. Go to the link above and print out the form for your child's grade level. A hard copy of this form must be returned to Kevin McMahon in Student Services.
  • The 2021-22 Annual Notice of Survey, Records, Curriculum, Privacy, and Related Rights and Opt-Out Forms
    • These are various forms related to your child's records and privacy that are located on the main FCPS website.
    • The link above provides instruction on how to access and fill out these forms.
    • These forms are good for the 2021-22 school year only. If you filled out these forms in a previous year, you will need to resubmit.
  • Free/Reduced Application information and forms for qualified families
    • While breakfast and lunch will be provided for free during the 2021-22 school year, we encourage you to sign up for this if eligible as it will make you eligible for other reduced fee programs. 

 


9th/10th Grade and New Student Orientation

  • We are excited to welcome 9th and 10th graders to Orientation on Thursday August 19, 2021.  Our day will begin at 8:10am and conclude at 12:00pm.  Our schedule for the morning is below.  A reminder schedules will be available beginning on August 19 and will go live in ParentVue and StudentVue at 6am that morning.  Students will report to their 3rd period class at 8:10am to start the day and will receive more information when they arrive at 3rd period (Advisory/Homeroom) class.  
  • All new 11th and 12th graders for the 21-22 school year AND 10th and 11th graders who were new to WPHS last year (20-21) school year are welcome to attend an upper class New Student Orientation on 8-19-21.  If you plan to attend, please complete the RSVP and join us in the library at 8:10am.
  • Transportation – While all the regular bus routes will not be active, we have arranged for multiple bus stops including at every elementary feeder school.

Wolverine Welcome Open House and Resource Fair - August 19

  • We are excited for the return of our annual Back to School Kick Off event – Wolverine Welcome.  It will occur on Thursday, August 19 from 3:30-5:30pm.
  • What can you do during Wolverine Welcome?
    • Attend Student Activities/Club/Sports Fair - Aux Gym
    • Visit WPHS Resource Fair – Main Gym          
      • Confirmed resources include: Gum Springs Community Center, Anthem Health Keepers Plus, FCPS Parent Resource Center, Financial Empowerment Center, South County Teen Center, FCPS Department of Family Services, United Healthcare Community Plan, Second story, Post 2252 Fairfax County Public Safety Cadets and Commonwealth ChalleNGe Youth Academy.
    • Pick up your child's laptop
    • Walk the building to find classrooms and lockers (All 9th and 10th graders are issued a locker)
    • The Clinic will be available to drop off any medication
    • Foods Services available to add money to lunch accounts
    • Sign up to receive a Fairfax County Bus Pass
  • After Wolverine Welcome, beginning at 6:00pm our Student Services team will be hosting Class of 2022 College Planning Night in the Springbank Building.

Laptop Distribution 

  • There will be several opportunities to pick up your student laptop prior to the first day of school.  All students can begin picking up their laptop tomorrow 8/18.  We will also be distributing laptops during Orientation and Wolverine Welcome.  Students who are unable to pick up their laptops during the summer distribution will be able to pick up their laptops during the first week of school.

Some important things to keep in mind:

    • Enter through Door 13 (gym lobby).  (Keep in mind that the building is currently under construction. You may need to park in the junior/senior lots and walk to door 13).
    • Students will be issued a laptop and charger.  Read more about care, lost or damaged procedures and potential fees here.
    • Laptops will only be issued to enrolled students who are present (you can not pick up a laptop for another student). 
    • Before students take the laptop home, they will log in to the computer to make sure the passwords are up to date- plan for approximately 45 minutes for this process, including potential wait and troubleshooting time. 
    • Students are expected to bring a charged laptop to school daily (including the first day!)
  • Now is a good time to begin talking to your student about how this laptop will be used to enhance their learning and discuss routines such as charging and safely storing the laptop.  FCPSOn family resources can be found here.

Parental Consent for Software/Applications

  • As part of our commitment to protect student data, FCPS is required to secure parental permission for student use of software/applications for which FCPS does not have a confidentiality agreement.  For ease of access, an online process has been developed to collect this information digitally and provide school-based reporting.
