TJ ANNUAL ATHLETIC PHYSICAL EVENT AND ATHLETIC REGISTRATION
Dear TJ Parents and Students
TJ and Westfield HS have teamed up this year to host the very popular Annual Sports Physical Event on Tuesday July 27th at Westfield HS. Appointments are available from 5:30-7:30. Medical professionals will be available to complete the VHSL January 2021 physical forms. The cost is $50 and all the money from TJ students comes directly to TJ Athletics! To register, go to

We are still looking for medical professionals to help at this event. Any MD, DO, PA, NP, RN, or PT is welcome to volunteer. If interested, please email hrmurphy@fcps.edu
REQUIRED PAPERWORK FOR OUT OF SEASON CONDITIONING & TRYOUTS

VHSL Physical Form-Must be the January 2021 version
- Must be dated on or after May 1, 2021
- New every school year
- Form is on our website under FORMS

DUE at the time of out of season condition events, or July 29th for FALL

Athletic Registration-ONLINE
- Must be completed every school year
- ONLINE: website “Athletic Registration”
- Indicate all sports for the 2021-2022 school year
- Complete concussion education
- Complete emergency care card
- Complete weight room permission, field trip driver’s information, & insurance
- Physicals from 2020-21 school year expire June 30, 2021. Students wanting to do out of season conditioning from July 1, 2021 on will need a NEW physical.
It is at the coach’s discretion if they hold out of season conditioning. A physical will be due that time. If you have turned in your physical to Heather Murphy, students will be added to a compliance report. If not, please give it to coach on on your your first day. Physicals are due PRIOR to tryouts each season July 29 for Golf and August 2nd for Football, Field Hockey, Cheerleading and Cross Country.
There will be no more sports passes. Coaches have access to a compliance report that indicates students have turned in a valid VHSL physical & competed athletic registration.
