Parents who have indicated that their child will attend in-person instruction must complete a Health Screening Commitment form. If you have previously submitted this form you do not need to complete it again.
This may be done via hard copy that parents must print, sign and then return either via email, mail or in person during our Materials Distribution and return on Mondays. If you choose to email the signed form, please send to Christie Armstrong at cparmstrong1@fcps.edu .
If your child will be returning to the building this one time form must be completed.
