Return to School - Required Documents

Please make sure you have completed all of the required forms below for each student as we begin this school year.

Emergency Care Form via weCare@school

Information must be correct and what we will use in case of an emergency. We are asking all parents to review and update via weCare@school for each enrolled student. Having the most up-to-date information for all of our students is essential to our ability to best meet all students' needs in the event of an emergency.

Health Information Form

A health information form must be on file for all students. By updating “Current Health Conditions” and/or “Current Medical Care Details” in the emergency care form via weCare@school, this will be updated for the student.

Student Rights and Responsibilities (SR&R) Signature Page

The Student Rights and Responsibilities document explains expectations for student behavior and adult responses to enhance school safety and to create a fair, equitable and supportive school environment. We ask that you use this document as a framework for discussing these expectations with your child or teen so that he or she is well prepared for success when the school year begins. You will also find that this publication includes information that you may need throughout the school year, including contact numbers and other resources.

Online access to review the FCPS SR&R is available here: https://www.fcps.edu/srr

Use this electronic link to the FCPS Student Rights and Responsibilities Parent Acknowledgement form. All parents and/or guardians must complete the acknowledgement form indicating they have reviewed and are familiar with the various rules and regulations that guide student conduct in FCPS schools. The form must be filled out one time per student.  Students will be reviewing the SR&R in 3rd Period Advisory (Learning Seminar) on Thursday, September 17.

Parent Digital Tool Permission

This information is provided in compliance with the Family Educational Rights and Privacy Act (FERPA) to provide you with information about the measures FCPS takes to keep student data secure when using digital tools. By completing this digital form, you provide permission for your child to use the FCPS-approved online tools listed below as part of an instructional program.

More information for you to review regarding what digital tools your children may be using this year can be found here.  Parents only need to complete this form one time and it will cover all classes.

National Honor Society Update

For all students interested in applying for NHS this school please be patient as we prepare and process our first steps in the coming weeks. Only current Juniors or Seniors with a 3.5 Cumulative GPA will be eligible to apply. Please wait to email or contact school personnel, as we prepare our application and submission process. If students are eligible to apply they will receive notice via their school email in a few weeks. All current members should continue to complete service hours as best as you can. We will hold office election nominations in the coming weeks as well as our first meeting of the year. Again, please be patient as we navigate a busy virtual opening to our school year.

AP Class Information

If you are taking an AP class this year, please read below for important information! Please make sure you can access your AP account!

All students will have to have a College Board account in order to access important classroom materials as well as register for SAT exams and view scores for SATs, PSATs, and APs. This needs to be an account that students can regularly view and should have access to by September 11. Students should use their personal email address and not their school email address, as the school email address will expire when they graduate and will not be able to receive emails from the College Board.

If you don’t already have an account, please sign up at myap.collegeboard.org. Parents shouldn’t create an account for their student. Students, please keep your account information in a safe place only you can access.

Students will login here: myap.collegeboard.org. You can either log in to make sure you can access your account or create a new account there. If you are having issues accessing your student account, visit the College Board’s help site.

Our OHS student portal also has links to AP-related information and resources for you to use throughout the year.

If you have issues accessing your College Board account, there are prompts on that page that can help you, you can contact the College Board at 888-225-5427, or visit the College Board’s help siteOakton High School is not able to help with College Board accounts.

All paperwork is to be given to Shelley Wiederhold, SSD Coordinator at Oakton High School. Each of the following must be completed:
