Every school year we collect vital information from our Olde Creek families during the first few days and weeks of school. A packet of these forms were sent home in the student materials bags yesterday. If you were unable to pick up your child's material bag, please feel free to stop by the office Monday-Friday between 8:00am-4:00pm. Please note: The office will be closed for the Labor Day holiday beginning Friday, 9/4 and will reopen Tuesday, 9/8.

How to Submit Forms
Below you will find links for downloadable copies and some digital submission options for these important forms. If you are able, we encourage your family to complete and submit the forms digitally (when possible).
If you're unable to submit the forms digitally, please feel free to drop them off at Olde Creek during the hours of 8:00am-4:00pm. When you arrive, please ring the doorbell and let the front office know that you're dropping off forms.
Required Forms
The Health Information Form and the Emergency Care Information Form provide valuable information regarding student health conditions. These forms are critical in the early identification of students who may need support in the educational environment. Along with the development of Healthcare Plans (HCP) and Action Plans, the Public Health Nurses utilize the information contained in the forms to develop teaching strategies for FCPS staff. Even though we are beginning the year virtually, it is critical that all forms are completed. These are available for electronic submission by parents via their FCPS 24/7 (Blackboard) accounts within the weCare@school (weCare) module. Emergency Care Card Form
- If possible, you make any changes using weCare@School via your FCPS 24/7 Parent View account. We must have an updated form for every student on file at the school. It is extremely important that we have the name and phone numbers of at least two people in the “Other Contact Information” section of the form. Please be sure to notify the school office of any changes in phone numbers or email addresses that occur during the school year by using weCare@School via your FCPS 24/7 Parent View account.
Health Information Form (Grades 1 - 6 ONLY)
- This form needs to be completed annually to inform us about your student’s health condition that affects his or her school day. Please sign and date prior to returning. This form is available for electronic submission by parents via their FCPS 24/7 (Blackboard) accounts within the weCare@school (weCare) module.
Students Rights & Responsibilities Acknowledgement Form
- Please review the Student Rights and Responsibilities and related resources at www.fcps.edu/srr. After reviewing the information, fill out the SR&R Parent Acknowledgement Form using this link. The form must be filled out one time per student.
Parent/Guardian Consent for Select Digital Resources
- At Olde Creek Elementary School, our teachers will use a variety of digital resources to support student learning and promote communication and collaboration with students and families. Some of the digital resources your child may use this year will require your consent before they can be used. You can find our school’s list of the resources that require parental consent here.
- Included on this page is also a link to a form where the enrolling parent or guardian will need to indicate whether or not their student is allowed to use the list of digital resources this year. This page also provides more information about how FCPS carefully reviews digital resources for approval and why we must request your consent before using certain tools. We encourage you to complete the parental consent for digital resources form as soon as possible, as your child will not be able to use the digital resources until we have received your permission. Please reach out to the school if you have questions or concerns about the digital resources we’ve chosen to use this year.

CPS Annual Notice of Survey, Records, Curriculum, Privacy, and Related Rights and Opt-Out Forms - This booklet provides you with detailed information about your opt-out choices. The form is due no later than the Friday of the 2nd week of school and includes:

Family Life Education Instructional Program Information & Opt-Out Forms
Fairfax County Public Schools (FCPS) provides a comprehensive, sequential Family Life Education (FLE) program for students in grades kindergarten through grade twelve. Instruction is seen as a partnership among parents and guardians, the school, and the community in supporting the learning essential to the development of strong families, positive relationships, and a healthy community.
Parents/guardians may choose to opt their child out of all or part of the Family Life Education program. Students who are opted out are provided with age-appropriate, nonpunitive alternative health instruction, and every effort is made to foster respect for family choices. If you wish to opt your child out of all or part of FLE for this school year, opt out forms have been included in your child’s back-to-school/orientation packets and are available online for Elementary (K-6) here.
Family Life Education Opt Out Forms
Please read and return the opt-out form for your child’s grade level ONLY if you do NOT want your child to participate. Program questions may be addressed at our school by contacting our assistant principal, Alison Butler. If you choose to have your child participate in the Family Life Education Program, you do not need to return the form.
Detailed grade-level program descriptions are available online at Elementary (K-6). FLE grade level-specific lessons and media that FCPS has streaming rights for are available for parents online in FCPS 24-7 (Blackboard) Parent View. Account information is available at your local school or online here. Grade level-specific lessons are also available at the Fairfax County Regional Library in the City of Fairfax (this location does not have media). Grade level-specific lessons and media that is not streamed online are available for review at your child’s school library. Due to copyright permissions, we are unable to stream all media online.
