BROOKFIELD ES PARENT REMINDER

PICKING UP STUDENT MEDICATION AT THE END OF SCHOOL YEAR 2018-19

  • All student medications must be picked up by a parent no later than 1:00 PM on June 13, 2019.
  • Medications may also be picked up prior to June 13: M-F 8am-3pm.
  • Medication left in the Health Room after 1 pm on June 13 cannot be saved and must be discarded according to FCPS and FCHD policy.
  • REMINDER: New medication authorization forms are required for each new school year.  Forms can be located at:
