 Your care coordinator is a resource to help you make sure you are getting the health services you need. Your care coordinator works with you to create a personalized care plan to meet your health needs and preferences, and helps answer questions about your health care. Care coordinators can also:
- help you find a new provider or specialist;
- help you access needed community resources and social services;
- improve communication between your providers through care team meetings;
- monitor your progress toward meeting your goals.
