Your care coordinator is a resource to help you make sure you are getting the health services you need.  Your care coordinator works with you to create a personalized care plan to meet your health needs and preferences, and helps answer questions about your health care. Care coordinators can also:

  • help you find a new provider or specialist;
  • help you access needed community resources and social services;
  • improve communication between your providers through care team meetings;
  • monitor your progress toward meeting your goals.
