The Department of Medical Assistance Services (DMAS) works with six different health plans to provide your Medicaid health coverage. Commonwealth Coordinated Care Plus, or CCC Plus, is one program within DMAS that serves members with complex care needs. For more information, visit our website at www.cccplusva.com.

As a CCC Plus member, you were either assigned to a health plan, or you selected one that best meets your needs. The information in the member materials explain the six health plans that are available to you and the different added benefits they offer. 

Once a year, you have the option to change your health plan for any reason during open enrollment in the fall. You will receive notice in the mail when it is time for open enrollment. 

Your Member Handbook is a great tool to help answer many of your questions. This handbook is located on your health plan’s website and includes valuable information such as how to get care, services and prescription drugs, where to go for help, how the CCC Plus program works, and much more.
