Series Description:

Post-Traumatic Stress Disorder (PTSD) is one of the major disorders seen in Veteran patients by VHA clinicians. New information and recommendations must be communicated to all health care providers to keep up with the continuous updates in this field. With this knowledge-based monthly teleconference series, experts will provide VHA staff with various approaches to treatment and other relevant information regarding PTSD.

Lecture Description:

Credit/hours: 1 Credit Hour

Often clinicians struggle to identify residential treatment resources for Veterans diagnosed with PTSD, particularly for those Veterans with co-occurring substance use and/or other mental health concerns. This struggle often is related to a lack of awareness of how best to access information about the residential treatment programs and a lack of understanding of the current standards for mental health residential rehabilitation and treatment in VHA. This presentation will highlight strategies for ensuring seamless access to residential treatment for Veterans diagnosed with PTSD. The presentation will focus on the role of residential treatment in the overall treatment continuum, identifying for whom residential treatment would be indicated and strategies for identifying programs and facilitating a referral. Current, up-to-date Best Practices for ensuring a seamless transition from outpatient treatment to residential care and subsequently back to outpatient treatment will be discussed.  

Speaker:

JENNIFER BURDEN, PhD., is the Deputy Director, Mental Health Residential Rehabilitation Treatment Programs (MH RRTP) in VACO Mental Health Services (MHS).  Prior to joining MHS, Dr. Burden served as the Network Substance Use Disorders Services Coordinator for VISN 6.  Dr. Burden has worked in VA for almost 13 years beginning her career at the Salem VA Medical Center as the project director for an HSR&D funded clinical trial before becoming the Program Director of a 10-bed PRRTP for Veterans with co-occurring substance use disorders and mental health concerns. Dr. Burden completed her graduate training in Clinical Psychology at Syracuse University. As the  Deputy Director, MH RRTP,  Dr. Burden’s primary responsibilities include supporting efforts related to the ongoing national policy planning, design, development and implementation of the Mental Health Residential Rehabilitation Treatment Programs which includes Domiciliary Residential Rehabilitation Treatment Programs (DRRTP), Domiciliary Care for Homeless Veterans (DCHV), Psychosocial Residential Rehabilitation Treatment Programs (PRRTP), Substance Abuse Residential Rehabilitation Treatment Programs (SARRTP), PTSD Residential Rehabilitation Treatment Programs (PRRP) and the Compensated Work Therapy/Transitional Residence (CWT/TR) Programs. 

Audience:  

The target audience includes psychiatrists, psychologists, registered nurses, physicians, certified/licensed counselors and other staff providing mental health services to Veterans with PTSD.

Series Description:

The issues of polypharmacy and guideline-concordant prescribing remain a priority for VA in Veterans diagnosed with Posttraumatic Stress Disorder (PTSD). This knowledge-based lecture series will provide information on pharmacologic best practices in patients with PTSD and updates to recommendations in the PTSD Clinical Practice Guideline. With PTSD being one of the major disorders seen by VHA clinicians in Veteran patients, new information and recommendations must be communicated to all health care providers to keep up with the continuous updates to prescribing practices, practice guidelines, and pharmacologic best practices.

Recent research has determined that despite the VA/DoD Clinical Practice Guideline recommendation against their use, prescribing of benzodiazepines for Veterans with PTSD remains above 30% in the VA.  This suggests that there is a gap between guideline recommendations and actual clinical care.  Because of renewed interest in policies regarding benzodiazepine use and the growing population of Veterans with PTSD seeking mental health treatment, the topic of strategies to decrease benzodiazepine use in PTSD presents a significant opportunity for improvement in the quality of clinical care.   

Research has focused on characterizing benzodiazepine prescribing trends in VA for Veterans with PTSD over the past decade.  Using VA administrative data, investigators examined outpatient prescriptions for all Veterans with PTSD including first line recommended antidepressants, benzodiazepines, atypical antipsychotics as well as Prazosin, used to treat PTSD-related sleep problems.  Findings from this work as well as more recent published research that has examined demographic characteristics of those Veterans who receive benzodiazepine prescriptions as well as the location of the prescribing clinician’s clinic will be reviewed.  As providers noted a need for education and suggestions as to how best taper a Veteran from these medications, a plan to achieve optimal clinical outcomes for Veterans with PTSD will be discussed.   

This virtual conference will focus on identifying the first line recommended pharmacotherapies for patients presenting with PTSD, determining subgroups of patients with PTSD where benzodiazepines are clearly contraindicated, identifying alternative treatment strategies for specific symptoms, as well as how to collaborate with other clinical staff and your patient to reduce the use of benzodiazepines in Veterans with PTSD.  

Speakers:

TASHA SOUTER, M.D. completed her undergraduate degree in Psychobiology at Claremont McKenna College and received her M.D. from the University of California San Francisco.  She completed her Internship and Residency training in Adult Psychiatry at Stanford University Medical School.  She is a Diplomate of the American Board of Psychiatry and Neurology.  Dr. Souter is currently the Medical Director of the Trauma Recovery Program at the VA Palo Alto Health Care System.  She is also a Clinical Instructor (Affiliated) at the Stanford University School of Medicine, Department of Psychiatry and Behavioral Sciences.  She provides and oversees psychiatric and medical patient care for the 40 bed Men's Trauma Recovery Program and the 10 bed Women's Trauma Recovery Program, the VAPAHCS's residential rehabilitation units for the treatment of PTSD.  Dr. Souter is a mentor, teacher, and supervisor for Stanford Psychiatry Residents.  Her areas of interest and expertise include the evaluation and treatment of combat related trauma, military sexual trauma, women's mental health, suicide, OIF/OEF/OND and Active Duty issues, teaching psychiatry residents psychotherapy, provider skills related to treating patients with severe trauma histories, and provider self-care.

NANCY BERNARDY, Ph.D. is a clinical research psychologist at the Executive Division of the National Center for PTSD in Vermont and an Assistant Professor, Department of Psychiatry, The Geisel School of Medicine at Dartmouth.  She directs the national PTSD Mentoring Program; a program that shares best administrative practices in PTSD specialty clinics to improve the delivery of quality PTSD care for Veterans.  She is a funded VA MH QUERI researcher and her work has focused on improvement of the quality of treatment of Veterans with PTSD in VA through appropriate prescribing of medications and access to evidence-based psychotherapies.

  1. Click on the Item Link above.  You will be taken to the TMS Login page. 
  2. After logging in you will see the Item Detail page for this program.
  3. On the right side of the page there are 3 buttons: “Go to Content,”  “View Offerings” and “Add to To-Do List.” 
  4. Select the “View Offerings” button.  (Do not select the “Add to To-Do List” option.  This will add the training to your To-Do List, but it does not register you for the training, and you would not be able to take the evaluation or post- test to qualify for CEUs.)
  5. After clicking on the “View Offerings” button, the date, time and location for the scheduled offering will appear.
  6. Select this program by clicking on the “Register” button to the right of that offering.
  7. On the next page, you will click on the “Confirm” button on the upper right side of the page. This will register you and add the training to your To-Do List and you will receive an email enrollment confirmation from TMS.
