Tele Whole Health focuses on you!
Whole health centers around what matters to you, not what is the matter with you. This means your health care team will get to know you as a person and develop a personalized health plan based on your values, needs, and goals. The road to better health and well-being starts here, with you.
Our VISN 4 Clinical Resource Hub (CRH) offers numerous programs to help support your whole health and well-being from the comfort of your own home. If you are interested in one of our specialized offerings listed below, contact your VA primary care provider for a referral to VISN4 CRH WHOLE HEALTH.

We recently had a new team member join the CRH Tele Whole Health Program. Maureen Brackey, LCSW (she/her), completed her undergraduate degree in Communication Studies and Mass Media Marketing at UCLA and her MSW at University of Southern California, with a concentration in Military Social Work and trauma recovery.
In 2017 Maureen transitioned to a career in VA as a behavioral health interdisciplinary program social worker at the Lebanon VA Medical Center. Maureen also took on the LGBTQ+ program as a collateral duty which she grew to a full-time role by 2023.
Maureen's clinical focus areas include trauma, relationships, sexuality, gender identity, the personality disorder spectrum, and whole health/wellness. She joined the VISN 4 clinical resource hub in March 2024 as a whole health social worker and will continue in her role as LGBTQ+ program lead in VISN 4. She is also excited to be developing a clinic for the provision of sex therapy and education on sexual health and wellness.
