Goals of Care Conversations
The VA’s Life-Sustaining Treatment Decisions Initiative (LSTDI) is designed to improve patient-centered care for Veterans with serious illness. The training promotes proactive goals of care conversations (GoCC) with patients who are at high risk of a life-threatening clinical event, and provides new processes for documenting the discussion and decisions in an easy-to-find progress note and orders in CPRS. This initiative is aligned with VA ICARE values, and will help us elicit, document, and honor our patients’ values, goals, and preferences as they face serious illness.
This 8-hour, live virtual training is designed to prepare clinicians with the tools needed to have conversations with patients and families about end-of-life decisions while they have the capacity to share their decisions. Practice sessions are included and resources are provided.
Audience: The main audience for this training are: Social Workers, RNs, Psychologists and Chaplains. Other direct patient-care staff are welcome to attend.

Accreditations: 7 CEs are provided for JA IPCE, ACCME, ANCC, NBCC, APA, ASWB, CDR
This course will be offered using Microsoft Teams. A Teams Meeting Invite will be sent prior to the day of the class including the link to this Training.
Your desktop / laptop must have audio speakers and preferably a working microphone in order to participate in this training.
Objectives:
At the conclusion of this educational program, learners will be able to:
- Interpret the purpose of goals of care conversations;
- Demonstrate key communication skills for goals of care conversations;
- Relate how Goals of Care Conversation Training supports the Life-Sustaining Treatment Decisions Initiative;
- Show the six steps of a complete goals of care conversation: Introduce the Conversation, Identify Surrogate, Assess Understanding of Health, Eliciting Goals of Care, Discuss Services and Treatments, Summarize and Establish Follow-up Plan;
- Apply methods for documenting goals of care conversations in the health record;
- Use team-based strategies for successful implementation of goals of care conversations; and
- Show strategies for engaging learners and responding to challenging teaching situations.
