Program Description:
“Super utilizers” is a term that refers to the small percentage of patients who consume a disproportionate share of all acute care service utilization in a healthcare system. Homelessness is a robust social determinant of super utilization of acute care. In the Veterans Health Administration, data analytics and specialized medical home models have been developed to target and tailor care to homeless veterans who are super utilizers of acute care; however, these services lack high-value interventions that are directly aimed at helping homeless veterans become more engaged in their professional care and self-care. This webinar will discuss the benefits and potential of using peer specialists trained in whole health coaching to meet the needs of homeless veterans who are super utilizers of acute. We will discuss the core elements of this approach, modifications to conducting Whole Health Coaching with homeless veterans, and patient and contextual factors that impact the implementation of this approach.
