We want to make sure you are aware that the Department of Health Services (DHS) is offering a $100 incentive to Wisconsin Residents that receive their first dose of a COVID-19 Vaccine at a Vaccination Clinic in Wisconsin between the dates of August 20, 2021 and September 6, 2021. Our records indicate that you have not been vaccinated and you may qualify for this incentive.

COVID-19 vaccines are safe, effective, free, and widely available. Everyone ages 12 and older is recommended to get a COVID-19 vaccine to help protect themselves and others from COVID-19. And if you get your first dose of any COVID-19 vaccine from a vaccine provider located in Wisconsin, including at the Madison VA, beginning August 20 through September 6, you get $100!

Who is eligible for a vaccination reward? 

Everyone who is eligible to receive the free COVID-19 vaccine is eligible to receive this gift card, regardless of insurance, ID, or citizenship. Just get your first COVID-19 vaccine from a Wisconsin vaccine provider between August 20 through September 6.

How do I get my vaccination reward? 

To get your $100 reward there are a few steps you need to take: 

• Get your first dose of any COVID-19 vaccine anytime between August 20 and September 6, 2021 from a Wisconsin vaccine provider. 
• Fill out the registration form so we can verify you received your first vaccine dose and know where to send the $100 Visa Gift Card. Parents and Guardians will need to fill out the form for children ages 12-17.   The registration form can be found at https://100.wisconsin.gov
• You will be asked to provide an email address and physical address. If you do not have one, please call 844-684-1064 and they will help you. Note: It can take up to six (6) weeks for you to get your Visa gift card in the mail.
• Those vaccinated at the VA will need to fill out a release of information form to allow us to confirm with the state that you received your vaccination and are eligible for the incentive.  Please fill out the attached form and return to your VA Vaccination Clinic in person or by mail. You may also fax the form to 608-830-6655.
