Jansen Only Vaccination Clinic VA SORCC Building 211A Tuesday April 20th and 27th 4pm-7pm
VA SORCC is now offering COVID-19 vaccine to all Veterans, Their Spouses, Caregivers of Veterans, surviving spouse of a Veteran or Recipients of Civilian Health and Medical Program of the Department of Veterans Affairs (CHAMPVA) benefits. Call 541-826-2111 extension 4440 between the hours of 8:00 a.m. to 4:30 p.m. Monday through Friday to schedule a single dose Jansen COVID-19 vaccine appointment on one of the dates above. We cannot take walk-ins. If you are not an enrolled Veteran with VA SORCC, please fill out and bring the attached COVID-19 VACCINATION WORKSHEET with you to your vaccination appointment.
Enrolled Veterans, if you’ve been vaccinated in the community, please let us know. Take a picture of your vaccination card and send it to your care team via MyHealtheVet or stop by your nearest VA clinic and ask them to scan it to your medical record.
