SALISBURY, N.C. – COVID-19 vaccines are now available for Veterans over 65 at the Salisbury VA main campus and Charlotte and Kernersville Health Care Centers. If you are an eligible Veteran over 65, please call (800) 706-9126 to schedule your appointment. In the future, we will continue to offer COVID-19 vaccinations to additional age groups.
Veterans and staff are encouraged to take every day preventative actions to avoid being exposed to the virus:

FYI, Salisbury Health Care System Veterans: As part of our ongoing efforts to ensure the safety and well being of our Veterans and staff, beginning Wednesday, March 24, 2020, routine face-to-face services will no longer be available for Eligibility and Release of Information at any of our sites. Telephone customer servcie will continue as usual and alternative service options can be found below. Emergency requests will be considered and addressed on a case-by-case basis at each site. Thank you for your support and understanding during this challenging time: If you need to enroll for VA Healthcare, you can take a 10-10EZ form and a pre-paid envelope and mail your application to the Salisbury VAMC Eligibility Department at: ATTN: ELIGIBILITY (136C) OR To register by telephone, you ay contact the HEALTH RESOURCE CENTER at 1-877-222-VETS (8387) OR You can go online and enroll using the link below: https://www.va.gov/health-care/apply/application/introduction Beneficiary Travel Claims will be processed at the clinic where your appointment was conducted. In order to continue to provide Veterans with the option to request their medical record for themselves or another organization, the following options will be available: 1. Call the Release of Information Office at (704) 638-9000 ext. 12610 or 12601 to speak to an ROI Clerk to get help. 2. To request a copy of your medical records to be mailed to yourself, complete the authorization form 10-5345a. You can access the authorization form online: https://www.salisbury.va.gov/patients/roi.asp Resources box: VA Form 10-5345a Individuals' Request for Copy of Their Own Health Information 3. To request a copy of your medical records to be mailed or faxed to another organization such as an insurance company, attorney, Social Security Disability, etc., complete the authorization form 10-5345. You can access the authorization form online: https://www.va.gov/vaforms/medical/pdf/10-5345.pdf There are a few options request your records: Fax the completed and signed authorization form to (704) 645-6279. Mail the completed and signed authorization form to: VA Medical Center Drop Box will be available on-site in area. Sign in and/or Register to use MyhealtheVet to access portions of your electronic health records onloine and/or upload your completed and signed authorization. https://www.myhealth.va.gov/mhv-portal-web/home Non-VA Provider - For treatment purposes and to avoid delays with your care, your non-VA provider can fax or send a written request to us. We will submit the medical records directly to your provider. VA and Non-VA Forms Completion: Authorization form 10-5345b will need to be completed and signed. Both the authorization form and the form ofor our provier to complete must be submitted to us. The forms can be mailed or faxed or placed in the drop box.
