In response to a recommendation that the USPTO consider increasing and strengthening the solemnity of verified statements regarding use of a mark, the USPTO is reformatting the declaration and signature portion of the forms for applications for registration, allegations of use, and post-registration affidavits or declarations of continued use or excusable non-use.  The reformatted forms separate out the clauses to improve readability and require the signatory to check a box next to each statement in order to validate an electronic submission.

In order for users to preview the new format and compare it to the current format before the forms are officially released later this year, the USPTO posted both versions on the IdeaScale® online collaboration tool on July 28, 2016.  Those wishing to comment on the new format and have their comments taken into account before the new format becomes official should do so by August 28, 2016.   For assistance on using IdeaScale, see the comprehensive user guide.
