The Department of Defense Instruction 6490.11 was initially released in 2012 and established policies and procedures for the management of mild TBI in the deployed setting. This included the clarification of roles, responsibilities, and a standardized plan of action for potentially concussive events. For the non-deployed setting, DHA more recently released the Defense Health Agency Procedural Instruction 6490.04. The DHA-PI 6490.04 guides providers through the concussion management pathway, using the Military Acute Concussion Evaluation 2 (MACE2) and Progressive Return to Activity (PRA) by TBICoE. This instruction applies to everyone in the DHA, including individual components and clinical facilities, military departments, and DOD medical personnel involved in the delivery of health care services to eligible beneficiaries.
