Annual IVES E-Signature Audit

Dear IVES Participants,

Please submit your annual IVES electronic signature audit to wi.ives.participant.assistance@irs.gov no later than March 15, 2022. Use "Annual E-Signature Audit - ######" for the email 'Subject'. See requirements below. Audits not received by March 15, 2022 will forfeit participation in the IVES electronic signatures program. Thank you for your cooperation.  

 

All IVES participants using electronic signature have agreed to the following:

  • Electronic signatures must use an independent party to audit and ensure all electronic signature requests meet all requirements
  • The audit findings should compare the audit log information against the Form 4506-C, 4506-T or Form 4506T-EZ to validate the name(s) and date(s).
  • The monthly sample size for review will be 125 from the electronically signed Form 4506-C, 4506-T or Form 4506T-EZ against the audit log.  All IVES participants will develop their own skip interval each month by dividing their total number of requests by the sample size of 125.  Each month the participant will select the first electronically signed request and apply the skip interval from there.  The participant can stop the selection process once 125 requests have been selected.  All participants will select a sample size of 125 requests, regardless of fluctuations in their monthly volume.  The only exception is for months where fewer than 125 requests were processed.  Participants should select and review all requests in the month for months where fewer than 124 requests were processed.
  • Findings must include monthly results as well as, the total annual computations. 
  • Annual audit findings must be submitted by January 31st of the following year unless an extension of time is made and granted prior to the due date. 
  • Submit report to the following e-mail address:   wi.ives.participant.assistance@irs.gov.
  • Upon request the audit logs and the Form 4506-C, 4506-T and Form 4506T-EZ will be made available throughout the year to the IVES HQ Analysts. 

IVES participants with a finding showing a failure to meet all requirements will not be allowed to continue using electronic signatures.
