Targeted
Performance Improvement Visits and HRSA’s Diabetes Quality Improvement
Initiative
Project
Officers for health centers with a significant number of diabetes patients with
an A1C value greater than 9 percent may soon schedule a targeted technical
assistance (TA) site visit to support improvement in diabetes outcomes. As a
part of the HRSA Diabetes Quality Improvement Initiative, health centers
receiving targeted TA visits this year will receive on-site consultation from a
clinical expert regarding their clinical program and Quality Improvement/Quality
Assurance activities with a focus on diabetes. During the visit, health centers
will identify three actions to improve performance and
input the action plan in the EHBs. Subsequently, Project Officers will monitor
health centers’ progress in implementing their action plans. Make sure your
health center registers for Action Plan privileges in the EHBs. Watch a video to learn more.
National

alone, more than 34,000 people in the U.S. received the gift of life thanks to those
who said “yes” to donation.
