Preparing Your Application Documents
Before you begin your National Health Service Corps (NHSC) Loan Repayment Program Application, we encourage you to gather the following documentation:
Required Documents

Proof of Status as a U.S. Citizen or U.S. National – A copy of a valid U.S. passport, passport card, U.S. birth certificate, or U.S Naturalization Certificate. (Note: A driver’s license or a Social Security card are not acceptable.)

Loan Documentation – Applicants will be required to provide two types of documentation for each manual loan that is being submitted for consideration: (a) an account statement and (b) a disbursement report. For additional information on what information needs to be included on these documents, please see pages 39-41 of the Application & Program Guidance.

Application Tip: If you have Federal loans, you can import them electronically using the National Student Loan Data System (NSLDS) to access your Aid Summary Report. You must register and receive a Federal Student Aid ID (FSA ID). Register and receive your FSA ID here. |
Additional Supplemental Documents (if applicable)

Disadvantaged Background Form – This document must be completed by a school official.

Proof of name change – You may submit a copy of your marriage license if the name on your loans or other documents is different than the one on your NHSC application.

Verification of Existing Service Obligation – If you have an existing service commitment, you must submit proof that it will be completed prior to the NHSC application deadline.

Payment History – Former NHSC participants must verify that all funds were used to repay the loans that were approved as part of the applicant’s most recent NHSC contract.

Geriatrics Certification – Upload a copy of the certificate of completion or diploma from your geriatrics training program.
