To provide an overview of “Health Impact in Five Years” (HI-5)
Initiative with examples from the field
Objectives of the
webinar:
- Describe the “Health Impact in Five Years” intervention in detail and why it
is important for social determinants of health
- Identify at least two evidence-based community
wide interventions that can accelerate health improvement in five years or less
- Address underlying factors that can promote or
prevent health and why social determinants of health (SDOH) are key factors in
health outcomes.
What is the HI-5
Initiative?
The HI-5 (“high-five”) or “Health Impact in Five Years
Initiative” is a tool that highlights non-clinical, community-wide
interventions with a proven track record. Each intervention listed is
associated with improved health within five years or less as well as reported
cost effectiveness and/or cost savings over the lifetime of the population or
earlier.
