Requirement to Include Adversary Cover Sheet with Complaint - Procedures and Form Updated
In accordance with Administrative Order 18-03, please note that the Adversary Cover Sheet has been modified as a fillable pdf and is now available as Local Form G-16 on our Website.
Also, the following procedures have been updated:

Notice of Removal
Cover Sheet for Adversary Proceeding
In
response to questions from our customers, please note that if you are
filing a complaint to object to the discharge, there is only 1 option to
complete on the Cover Sheet, code #41.
