OPEN ENROLLMENT for SAMBA Employee Benevolent Fund (EBF)
|
|
May 1, 2021 to May 31, 2021
SAMBA EBF is a not-for-profit federal employee benefit association provided to some Federal agencies, which includes Department of the Interior (DOI). Generally, this program is only available to elect within an employee’s first 60 days with a participating agency. Open enrollment with SAMBA EBF is rare, and not a part of regular open season for benefits. The SAMBA Employee Benevolent Fund (EBF) provides a lump sum death benefit payment to the beneficiary(ies) of a participating deceased DOI employee. The benefit is usually paid within 24 to 48 hours after notification of death. Your beneficiary(ies) can use this benefit to help sustain them while your other benefits are being finalized.
Annual Premium: There are two coverage options:
- $39 per year for a death benefit payment of $17,500, OR
- $78 per year for a death benefit payment of $35,000
NOTE: The annual premium will be prorated for mid-year enrollments. For June 1 through December 31, 2021, the prorated premiums are:
- $22.87 for a death benefit payment of $17,500, OR
- $45.73 for a death benefit payment of $35,000
This open enrollment is applicable to all PERMANENT full and part-time employees who are actively at work (not on official leave, active military duty, or leave without pay status).
