1. This ALCOAST announces the cancellation of REF (A), and the
promulgation of REFs (B) and (C) as part of the modernization of
the U.S. Coast Guard's Emergency Medical Services (EMS) program.
2. This update in policy incorporates a robust network of support
for EMS practitioners, tactical medicine personnel, alignment with
the operational needs of the service, and adoption of a substantial
oversight and quality assurance (QA) / Quality Improvement (QI)
program that will provide the necessary support to meet the needs of
the service, our shipmates, and the public.
3. The major additions to the Coast Guard's EMS program include:
    a. Credentialing of CG EMS practitioners to ensure they have:
        1) current EMS National Registry EMT (NREMT) certification,
        2) current BLS certification,
        3) affiliation with the state of Department of Homeland
Security (DHS) and agency of HSWLSC or TRACEN Petaluma in the NREMT
database,
        4) current Health Insurance Portability and Accountability
Act (HIPAA) certification,
        5) written authorization by Local Emergency Medical Advisor
(LEMA) for Scope of Practice, and
        6) completion of annual proficiency skills verification.
    b. Assignment of a Local Emergency Medical Advisor (LEMA) for all
personnel credentialed as part of the CG EMS system. The LEMAs will
provide medical oversight and documentation review for CG EMS
practitioners.
    c. Adoption of the current DHS Basic Life Support (BLS) and
Advanced Life Support (ALS) protocols for all CG EMS practitioners.
    d. The creation of a "CG EMT" competency code in Direct Access
for tracking and identification of those CG EMS practitioners who
have been credentialed to operate in the CG EMS system.
    e. Documentation of all EMS calls with the utilization of Form
CG-5214.
    f. The creation of a CG EMS community of practice page on CG
SharePoint located at:

    g. Generation of a standardized EMT medical equipment list
that has been approved by the Coast Guard's EMS Governance and
Advisory Council. This list will be posted on the community of
practice page (link posted in section 3.f). All credentialed CG EMS
practitioners will be authorized to procure and carry the equipment
on this list.
    h. Annual sustainment training for EMTs to demonstrate
proficiency through skills verification.
4. The major additions to the Coast Guard's tactical medicine
program include:
    a. Establishment of nine competencies in Direct Access to support
and track Tactical Combat Casualty Care (TCCC) certifications.
        1) "TCCCASM" for TCCC All Service Member (ASM) provider.
        2) "TCCCCLS" for TCCC Combat Life Saver (CLS) Provider.
        3) "TCCCCMC" for TCCC Combat Medic / Corpsman (CMC) Provider.
        4) "TCCCCPP" for TCCC Combat Paramedic Provider.
        5) "TCCCASMT" for TCCC ASM Trainer.
        6) "TCCCCLST" for TCCC CLS Trainer.
        7) "TCCCCMCT" for TCCC CMC Trainer.
        8) "TCCCCPPT" for TCCCCPP Trainer.
        9) "TCCCAF" for TCCC Affiliate Faculty who can perform
Train-the-Trainer functions.
These competency codes expire three years after the date of
certification in accordance with REF (D).
    b. Adoption of TCCC training as outlined in REF (D) Section 4 as
the Coast Guard Standard for tactical medicine.
    c. Documentation of TCCC cases in the CG will be documented on
form DD-1380 or TCCC After Action Report. TCCC AAR should be
submitted to the Joint Trauma System, ideally within 72 hours post
care, at:

    d. The creation of a CG tactical medicine community of practice
page on CG SharePoint located at:

    e. Finalizing a standardized tactical medicine equipment list
for tactical medicine trained personnel that will be approved by
the Coast Guard's EMS Governance and Advisory Council (GAC).
All personnel trained and current in tactical medicine at any level
will be authorized to procure and carry the equipment on the
standardized tactical medicine equipment list that is relevant for
their level of training and scope of practice. The tactical medicine
equipment list will be posted on the community of practice page
referenced in paragraph 4.d.
    f. Initial TCCC certification follows DHA curriculum and
requirements. Refresher/sustainment training for CG TCCC providers
is defined in REF (C).
    g. TCCC CLS and CMC practitioners will function under the
oversight of a Local Emergency Medical Advisor (LEMA). LEMAs will
provide written authorization of scope of practice.
