This demonstration will address the health care needs of the state's justice-involved population, advance the state's health equity priorities, and support the objectives of the Medicaid program by making sure incarcerated individuals with high physical or behavioral health risks receive needed coverage and health care services pre- and post-release into the community. By linking community-based Medicaid providers with justice-involved populations prior to release, the state will improve health outcomes for people with a history of substance use, mental illness, and/or chronic disease by increasing access to stable and continuous care. The federal comment period will be open from December 30, 2024, through January 29, 2025.
