Today, the CMS Center for Medicaid and CHIP Services (CMCS) issued a State Medicaid Director Letter outlining state requirements that will begin with Federal Fiscal Year (FFY) 2024 reporting based on the final rule for the Health Home Core Sets. The letter also provides updates to the 2025 Health Home Core Sets.
