CMS Releases Federal Fiscal Year (FFY) 2019 Core Set Chart Packs

Today, the Centers for Medicare and Medicaid Services (CMS) released the FFY 2019 Child and Adult Core Set Chart Packs. The Chart Packs provide an overview of reporting as well as more in-depth analysis on measures reported by at least 25 states (frequently reported measures) using visuals for each Core Set measure.  This year we are able to provide information on 23 frequently reported child measures and 25 frequently reported adult measures voluntarily reported by states.  See the links below for these new resources as well as the full set of Child and Adult Core Set Measure reporting resources:
