The Early Years Individual Support Plan (EYISP) has been replaced. From January 2025, when asked to conduct 'Information gathering' as part of the SEND Under 5 process, the new information gathering form must be used instead of the EYISP plan. The EYISP has been removed from our web pages and replaced with the new form and guidance materials. This new information gathering form is accompanied by two supportive documents, to help you complete the information gathering process. You can find these documents and guidance on our diversity and inclusion web page, under the 'information gathering form' section.
This information gathering form will be submitted to the Special Education Needs Assessment Team (SENAT) and used to decide at the Considerations Panel whether a child will go forward for an Education, Health, and Care Needs Assessment (EHCNA). If an old EYISP document is submitted to SENAT after Friday 31 January 2025, it will be rejected.
Please be aware the SENAT contact email address has changed, all information gathering forms must be submitted to EHCNArequest@westsussex.gov.uk.
