We are pleased to share the updated version of the Parent Declaration form which can be used from the Summer Term 2024.
You have explained how crucial the Parent Declaration form is to support your conversations with parents and signing up for the Early Years Funded Entitlements from April 2024, hence releasing this ahead of the Provider Declaration April 2024 - March 2025. The Provider Declaration is currently going through the approval process and will be published as soon as possible.
The current version of the Parent Declaration form must be used in the following circumstances:
- When a child starts accessing funded hours for the first time (all new starters for Summer Term 2024 onwards should use the form that has the last updated date as ‘April 2024’).
- When there is a change to any of the details on the form, including address details, change to sessions attended or number of hours claimed.
- When submitting a claim for Disability Access Fund.
- When submitting a new claim for Early Years Pupil Premium.
This form can also be used when signing up families for the new working families offer for 9 month olds from September 2024.
As a reminder, the Parent Declaration must be signed by both provider and parent before headcount day in order to be able to claim at headcount. The form is designed to be e-mailed/completed electronically so e-signatures are acceptable.
We have included a section to log any changes to a child/parent/carer's legal names and/or address, which must be kept attached to the original Parent Declaration form. However, if there are any changes to funded hours, we still request that a new Parent Declaration be completed in full for financial auditing purposes.
The forms must be retained by the provider, for the current financial year (April to March), plus two years from the completion date. The form must be made available at the request of West Sussex County Council officers or Ofsted inspectors.
