Feedback on findings from our SEND Quality Assurance work
Quality assurance (QA) activity across the SEND & Inclusion Partnership continues to be developed through the SEND & Inclusion QA Framework. This is a document that tells us how and when we will look at what our practice is like; this can be through feedback from you, staff and our partners and by looking at the quality of the work we do and then how this will inform practice developments.
Part of this includes a check of the quality of Education Health and Care Plans (EHCPs). This is completed by auditors (managers and practitioners) from across the partnership using an online tool.
In October 2024, 45 EHCPs were audited and the overall quality showed a 10% increase in the number of rated good or outstanding plans (see below).
 There has also been an improvement in the quality of sections D, H1 and H2 (social care needs and provision).
Feedback received from families, children and young people indicate that there is a positive impact on their lives from the services they receive.
There have been improvements in communication between agencies and work is happening to check the quality of education, health and care needs assessments, annual reviews and the journey of children and young people with SEND.

The next cycle of audits and quality assurance activity is underway including looking at the quality of our EHCPs, annual reviews, information on complaints and compliments and work with the SEND Family Help Team. SEND and Inclusion service improvement plans
SEND and Inclusion Strategic Improvement Plan
A SEND and Inclusion Strategic Improvement Plan has been developed to deliver the SEND and Inclusion Strategy, this has been co-produced with all partners including children, young people and their families.
Workstream groups have started work to carry out the actions in the plan with representation across social care, education, health and the voluntary/private sectors, children, young people, young adults, and families.
The plan will be shared on the Local Offer in April 2025, this will be updated with progress within the workstream groups every six months.

Short Breaks Offer
The Short Breaks Offer is being developed as part of the Transforming Children’s Services Program Board. The Short Breaks Project, led by Jazmine Walker (Head of Service), is responsible for delivering this program of work.
A Co-Production event, led by Keysha Cespedes (Designated Social Care Officer for SEND), took place with Voice4Parents on 3rd December 2024 with lots of Parent Carers in attendance sharing their ideas. The Project Groups plans to develop a tiered approach encompassing Universal, Universal Plus, Targeted, Specialist and Specialist Plus was shared and lots of positive feedback received.
Since then, £277,200 worth of funding has been secured to develop the Universal, Universal Plus, and Targeted. This includes the creation of a Short Breaks Coordinator role to work with community, voluntary, and private providers to develop a Disability Charter and minimum standards across the City so that parents can be confident about advertised SEND provision when accessing services.
A further co-production work with parents will take place to further refine and develop the Universal, Universal Plus, and Targeted offer ensuring short breaks groups and services developed and commissioned meet the needs of children and young people with SEND and Parent Carers. The project team look forward to continuing to progress the Short Breaks Offer.
