Are you a Carer looking after someone with a Learning Disability?

If so has your GP been in contact regarding the annual health check for the person you look after?

If not are you aware of the annual health checks that are offered?

If your surgery has been in contact and offered this service and you have declined – is there a reason why?

Would you be interested in taking part in a focus group that looks at health provision for people with learning disabilities?

Shirley Crook is undertaking research looking at health provision for people with learning disabilities. She would be really interested in hearing your views, especially regarding the GP annual health check.

Please email Shirley Crook at shirleydiane.crook@gmail.com
