 Mae Arolygiaeth Gofal Iechyd Cymru (AGIC) wedi cyhoeddi adroddiad (13 Rhagfyr 2022) yn nodi bod angen gwella'r wardiau iechyd meddwl yn Ysbyty Ystrad Fawr yn Ystrad Mynach.
Cwblhaodd AGIC arolygiad dirybudd o ddwy ward iechyd meddwl, Tŷ Cyfannol ac Annwylfan, ar dri diwrnod olynol ym mis Medi eleni.
Gwelodd yr arolygiad feysydd o arfer da lle dywedodd y cleifion wrth arolygwyr AGIC eu bod yn teimlo bod staff yr ysbyty yn ymrwymedig i ddarparu gofal diogel ac effeithiol a'u bod yn deall eu hanghenion a'u gofynion unigol. Fodd bynnag, nododd AGIC hefyd nad oedd pob aelod o'r staff wedi cael hyfforddiant diogelu gorfodol cyfredol, gan gynnwys trais ac ymddygiad ymosodol, er gwaethaf y ffaith eu bod yn ymdrin â digwyddiadau lle roedd yn rhaid atal cleifion yn gorfforol.
Nododd y tîm arolygu nifer o faterion y mae angen sicrwydd ar unwaith yn eu cylch a chyfeirir at y materion hynny yn yr adroddiad. Yn ystod yr arolygiad ar y safle, dywedodd y staff wrth yr arolygwyr y bu nifer o achosion lle y bu'n rhaid atal cleifion yn gorfforol ar y ddwy ward a arolygwyd, a bod aelodau o'r staff nad oeddent wedi cwblhau hyfforddiant digonol wedi bod yn rhan o'r achosion hynny. O ganlyniad, ni chafodd arolygwyr AGIC sicrwydd bod y staff a'r cleifion yn cael eu hamddiffyn a'u diogelu'n llawn rhag niwed ac anafiadau pe byddai angen atal claf yn gorfforol.
Nododd yr arolygwyr fod lefel uchel o staff asiantaeth yn cael eu defnyddio ar ward Annwylfan, gan roi pwysau dilynol ar y staff arferol, gan nad yw staff asiantaeth mor gyfarwydd â'r cleifion, ac y gall tasgau fel trosglwyddo gwybodaeth rhwng shifftiau gymryd mwy o amser.
Dywedodd y cleifion wrth yr arolygwyr fod y staff yn ofalgar, yn ymroddedig ac yn llawn parch a'u bod yn dangos lefel dda o ddealltwriaeth. Gwelodd yr arolygwyr dîm arwain cefnogol a nododd y staff eu bod yn fodlon ar lefel y cymorth gan y rheolwyr. Dangosodd adolygiad o'r cofnodion meddygol fod eiriolwr iechyd meddwl arbenigol ar gael i'r cleifion a oedd yn cynnig gwybodaeth a chymorth pellach wedi'u teilwra'n benodol at yr unigolyn.
Yn ystod yr arolygiad, gofynnodd AGIC i'r staff gymryd camau ar unwaith i sicrhau bod meddyginiaethau yn cael eu storio'n ddiogel. Ar ward Tŷ Cyfannol, gwelodd yr arolygwyr gwpwrdd a oedd wedi'i ddifrodi lle roedd amrywiaeth o feddyginiaethau yn cael eu storio, y gellid cael gafael arnynt yn hawdd. Nododd yr arolygiad hefyd nad oedd gan bob aelod o'r staff gardiau mynediad priodol, a oedd yn golygu y gallent gael eu cloi mewn ardaloedd a gorfod aros i aelodau eraill o'r staff eu rhyddhau. Pe byddai argyfwng, roedd yn rhaid i staff nad oedd ganddynt gardiau mynediad ddefnyddio'r gloch alw frys i gael cymorth i adael yr ardal. Dywedodd aelodau o'r staff arferol wrth yr arolygwyr eu bod yn gorfod gadael staff asiantaeth i mewn ac allan o ardaloedd y ward yn barhaus gan ddefnyddio eu cardiau llithro eu hunain, a bod hyn yn tarfu ar eu dyletswyddau.
Mae Bwrdd Iechyd Prifysgol Aneurin Bevan wedi llunio cynllun sy'n cynnwys cyfres gynhwysfawr o gamau gweithredu i gefnogi gwelliannau.

‘Mae'n galonogol gweld bod y staff yn gweithio'n galed i deilwra gofal at anghenion unigol y cleifion ar y ddwy ward yn Ysbyty Ystrad Fawr. Fodd bynnag, mae'r heriau sy'n cael eu hwynebu ledled Cymru o ran recriwtio a chadw timau o staff arferol, wedi arwain at ddibyniaeth ar staff asiantaeth a allai amharu ar y gallu i reoli risgiau yn effeithiol a darparu gofal diogel yn gyson. Nododd ein harolygiad nifer o welliannau a phryderon uniongyrchol lle roedd angen i'r gwasanaeth weithredu ar unwaith.

Adroddiad Cryno ar yr Arolygiad
 Healthcare Inspectorate Wales (HIW) has issued a report (13th December 2022) highlighting the need for improvement within mental health wards at Ysbyty Ystrad Fawr in Ystrad Mynach.
HIW completed an unannounced inspection of two mental health wards, Ty Cyfannol and Annwylfan, across three consecutive days in September this year.
The inspection evidenced areas of good practice where patients told HIW inspectors’ that they felt hospital staff were committed to providing safe and effective care and understood their individual needs and requirements. However, HIW also identified that not all staff had up-to-date mandatory safeguarding training, including violence and aggression, despite attending incidents requiring patients to be restrained.
A number of issues requiring immediate assurance were identified by the inspection team and are referenced in the report. During the onsite inspection, staff informed inspectors that there had been a number of incidents of restraint on both wards inspected, and that these involved staff who had not completed adequate training. Due to this, HIW inspectors were not assured that staff and patients were being fully protected and safeguarded against harm and injury in the event of restraint being required.
Inspectors noted the high use of agency staff on Annwylfan Ward, subsequently putting pressure on regular staff, as agency staff are less familiar with the patients, and tasks such as handovers can take longer.
Patients told inspectors that staff were caring, respectful, dedicated and demonstrated a good level of understanding. Inspectors witnessed a supportive leadership team and staff stated that they were happy with the level of management support. A review of medical records evidenced that patients had access to a specialist mental health advocate who provided further tailored information and support.
During the inspection, HIW requested immediate action was taken to ensure the safe storage of medication. On Ty Cyfannol Ward inspectors witnessed a damaged cupboard storing a variety of medication which could be easily accessed. The inspection also highlighted that not all staff had appropriate access cards, which meant that they could be locked in areas waiting for other members of staff to let them out. In the event of an emergency, staff without access cards had to use the emergency call bell for assistance to exit the area. Regular staff told inspectors they were continuously letting agency staff in and out of the ward areas with their own swipe cards, distracting them from their duties.
A plan has been produced by Aneurin Bevan University Health Board which contains a comprehensive set of actions to support improvement.
Chief Executive of HIW, Alun Jones said:
‘It is positive to see staff working hard to tailor care to patients’ individual needs on both wards at Ysbyty Ystrad Fawr, however, the challenges, felt across Wales, of recruiting and retaining teams of regular staff, have seen a reliance on agency staff which may be hampering effective risk management and the consistent provision of safe care. Our inspection identified a number of improvements and immediate concerns which required the service to take immediate action.
Aneurin Bevan University Health Board has already taken positive steps to improve following our feedback and have shown a commitment to addressing the concerns highlighted. We will continue to engage with them as they seek to tackle the challenges they face’.
