Mae Arolygiaeth Gofal Iechyd Cymru (AGIC) yn cynnal adolygiad lleol o Ymddiriedolaeth Gwasanaethau Ambiwlans Cymru, a bydd yn ystyried yr effaith y mae aros mewn ambiwlansys y tu allan i Adrannau Achosion Brys yn ei chael ar ddiogelwch, preifatrwydd, urddas a phrofiad cyffredinol cleifion yn ystod pandemig COVID-19.
Fel rhan o'r adolygiad, mae gennym ddiddordeb mewn profiadau pobl o ddefnyddio Gwasanaeth Ambiwlans Cymru. Os ydych chi, rhywun rydych yn gofalu amdano, neu aelod o'ch teulu wedi cymryd taith heb ei chynllunio i ysbyty mewn ambiwlans yn ystod y flwyddyn ddiwethaf, cwblhewch ein harolwg i gleifion.
Gellir hefyd argraffu'r arolwg a'i ddychwelyd atom yn:

Healthcare Inspectorate Wales (HIW) is undertaking a local review of the Welsh Ambulance Service Trust, and will consider the impact of ambulance waits outside Emergency Departments, on patient safety, privacy, dignity and overall experience during the COVID-19 pandemic.
As part of the review, we are interested in people’s experiences of using the Welsh Ambulance Service. If you, someone you care for, or a family member have had an unplanned journey to hospital in an ambulance within the last year, please complete our patient survey.
The survey can also be printed and returned to us at:
