ADASS Peer Review finds Telford & Wrekin Council's Adult Social Care Services ‘exceptional’

A report advising on the findings of a recent Peer Review Challenge which judged Telford & Wrekin Council’s Adult Social Care services as ‘exceptional’ will be presented to members at the council’s cabinet meeting on 27 May.

The peer review, which comprises a system of self regulation based on a sector-led improvement approach, was undertaken by a challenge team of senior sector Association of Directors of Adult Social Service (ADASS) figures from across the West Midlands.

Summarising their findings, the peer challenge team stated that

“Telford has exceptional adult social care operational delivery and partnership relationships and is in a strong position to position itself as the leader of the population health, economic restoration and maximising the digital inclusion offer at the local place”.

Midlands Partnership Foundation Trust (MPFT) Update:

Children and young people’s trailblazer scheme

MPFT were delighted to get local young people involved in designing logos for the Mental Health Support Teams who cover 50 schools across Shropshire, Telford and Wrekin.

Both the winning designs came from Telford pupils, Cole Jones, a Year 4 pupil from Sir Alexandra Fleming School, and Libby Hewlett, a Year 8 pupil from Madeley Academy

Trailblazer Scheme

Wellbeing Bus

Colleagues from across health, social care and the voluntary sector have begun exploring a mobile service which will visit communities across Shropshire, Telford & Wrekin. This could include a range of information and advice and services such as medication reviews, physical heath checks or immunisations.

The ‘offer’ on the bus can be tailored to the community it is visiting and/or current health and wellbeing priorities.

The planning team are currently looking into the practical aspects of putting a bus on the road and seeking support and input from across the community. If you have any ideas about services on the bus, where it could go or any other suggestions please contact: cathy.riley@mpft.nhs.uk

Maternal Mental Health

Shropshire, Telford and Wrekin has been selected to be an Early Implementer site for the new NHS Maternal Mental Health Services, as part of a testing phase taking place in all regions of England through 2021/22. 

Locally, New Beginnings is an innovative service in collaboration with Telford Council which aims to provide a relationship based, empathetic and trauma informed approach to support pregnant women whose unborn child is assessed as being at risk and requires a pre-birth assessment and ongoing support.  Psychologists will work with the Family Assessment Team to provide this service.

In addition the Shropshire Community Perinatal Team are currently developing and strengthening partnerships with Telford & Wrekin Health Visitors, Family Nurse Partnership and Telford IAPT perinatal champions, to support the referral pathway and working with partners across the Local Maternity system to explore innovation and improved access.

Community Mental Health Transformation

MPFT is currently working alongside partners to develop an ambitious plan for transforming mental health services. 

Following a successful bid to NHS England/NHS Improvement, partners including the voluntary and community sector, primary care, the new Integrated Care System (ICS), and both Telford and Wrekin and Shropshire Councils will be working together to look at new ways to meet the mental health needs of our communities.

Engagement sessions will be held with people with lived experience of mental health services, staff, voluntary and community sector representatives and other interested local people.

The Family Safeguarding Model

Family Safeguarding

What is it?

The Family Safeguarding Model has been designed by Hertfordshire County Council to tackle the challenges around Children’s Safeguarding and in particular the compounding factors to risk of harm known as the ‘trio of vulnerabilities’ (domestic abuse, substance misuse and mental health). Telford & Wrekin Council was successful in a bid to be one of a small number of authorities to implement the Family Safeguarding Model through the DFE’s Strengthening Families, Protecting Children Programme in 2019.

What difference does it make?

The Department for Education (DfE) evaluation published an evaluation (Nov, 2020) that shows that the Family Safeguarding Model is effective at keeping more children safely within their families and helping parents to achieve lasting change. It reported:

  • The number of children coming into care across the authorities involved has reduced by up to 30% (per 10,000 population) and Child Protection Plan numbers have reduced up to 46%.
  • Repeat domestic abuse call-outs to the homes of the families involved for Police fall by up to 64%,
  • Mental health crisis contacts reduce.

What does it include?

The model focuses on parental mental health, substance misuse and domestic abuse through the…

  • Creation of multi-disciplinary, co-located teams.
  • Investment in skills development to create change using Motivational Interviewing.
  • Improved information sharing and decision making through Group Case Supervision.
  • Single database for shared recording – Workbook recording space and framework to guide practice.

When does it start?

It will be going live next month (June 2021).

Shropshire, Telford & Wrekin Integrated Care System (ICS)

We are all part of the Shropshire, Telford and Wrekin Integrated Care System, or ICS – an integrated care system is when NHS organisations work in partnership with local authorities and other partners, to take collective responsibility for managing resources, delivering care and improving the health of the population they serve.

The ICS consists of:

  • NHS Shropshire, Telford & Wrekin Clinical Commissioning Group
  • Telford & Wrekin Council
  • Shropshire Council
  • Primary care/GPs
  • The voluntary, community and social enterprise sector
  • The Shrewsbury and Telford Hospital NHS Trust
  • The Robert Jones Agnes Hunt Orthopaedic Hospital NHS Foundation Trust
  • Midlands Partnership NHS Foundation Trust and
  • Shropshire Community Health NHS Trust
  • Healthwatch

We are on a journey to create a much more integrated health and care system across Shropshire, Telford and Wrekin, working as a multi-organisational partnership both in terms of planning and commissioning services across our population and in developing more integrated services on the ground.

The central aim of an ICS is to integrate care across different organisations and settings, join up hospital and community-based services, physical and mental health, and health and social care. This more joined up, integrated approach brings real benefits to patients.

Introducing a single commissioning group for Shropshire, Telford and Wrekin

NHS Shropshire, Telford and Wrekin Clinical Commissioning Group (STW CCG) formally came into being on 1 April 2021.

The move creates a single, strategic commissioning group for Shropshire,  Telford and Wrekin providing a unified approach to the planning, buying and monitoring of local health services.

The CCG will continue to work with a wide range of partners, including the health and social care providers, Local Authorities and voluntary and community sector organisations. 

The CCG will also work closely with patient groups and organisations, including Healthwatch, to ensure we have a two way dialogue with local people, so patients have a voice which is heard. This will help to ensure we understand the health needs of the local population and develop tailored solutions which create more equal access to services across the county, using our limited resources as effectively as possible.

TWIPP Integration Workshop: Older People focus

Thank you to everyone who attended the Older People workshops at the beginning of May.  Across the two sessions we had almost 70 people from 17 different organisations attend.  The aim of the workshops were to look at the ways in which we can work together to improve outcomes for older people in Telford and Wrekin. The workshops enabled attendees to share ideas and hear different perspectives with people feeding back:

“The workshop supported and developed my understanding of the needs of older people and generated a range of objectives to improve quality of life outcomes for older people in the Borough”

“I felt the workshop was very useful and very informative. I thoroughly enjoyed discussing topics with other professionals. Excellent workshop. Thank you”

The workshop outcomes are being collated and will be used to share the work moving forward.  Throughout the next stages of this work, co-production with people with lived experience, their families and carers, and those who work with and support older people will be a priority.

We received over 300 responses on how we all can better help our residents with a learning disability to live well & independently. Every response helps finalise a borough-wide place-based Learning Disability Strategy focusing on the most important improvements we and our partners will be making in the next five years.

We will share the consultation results & strategy with you as soon as they are finalised in July.

Celebrating Deaf Awareness Week

Deaf awareness week at the ILC

On Wednesday 5 May we celebrated Deaf Awareness Week at the Independent Living Centre (ILC) by inviting partners from both health and the voluntary sector.

Audiology came along to view the ILC and learn about the services that will be delivered from there for people with a sensory loss.

The strong relationship between Audiology and Adult Social Care is vital to ensure that people receive an end to end service and have access to the right information and support.

In April, Telford & Wrekin Council launched a brand new and innovative Virtual House.

The tour takes you through a home that has been set up with assistive technology, sensory and Occupational Therapy equipment. If the viewer identifies an item that they are interested in they can click on it to get further details.  This self-help tool will help residents to explore different equipment and ideas to help keep safe and independent at home.

Since it’s launch there have already been 2488 views!

Loneliness Awareness Week - get involved! (14-18 June)

Over the last two years Telford & Wrekin Council have supported the Loneliness Awareness campaign, developed by the Marmalade Trust. They are really interested in pulling together a programme for the week which really highlights what our residents can access to help them stay well and connected.
