What are the TWIPP priorities?

In Autumn 2020 the priorities were revised to reflect the changes and opportunities for further integration brought about through changes in delivery, opportunities developed through dealing with the COVID-19 pandemic and in light of the emerging Integrated Care System.  This update was agreed at the TWIPP by all members in December 2020. 

The TWIPP 6 strategic priorities are:

  1. Integrated care and support pathways: delivering joined up, effective services, providing support, and care, which connect and empower people to stay healthier for longer and support families to stay together, preventing avoidable admission to care homes, to hospital and preventing children being taken into care whilst making the most of the Telford £.
  2. Integrated advice, information and access to support: providing a comprehensive and integrated approach to a single point of access for health and social care, and providing information and advice for all ages, from health and social care services, to voluntary sector organisations, community groups, activities and support.
  3. Building community capacity and resilience: ensuring Telford and Wrekin is a place where all communities are well supported to take ownership of the challenges that they face, to make them stronger and more resilient.
  4. Integrated response to tackling health inequalities: working together to agree a ‘reducing health inequalities charter’ setting out the principles we will all adopt to ensure reducing inequalities is embedded in our strategic decision making, investment decisions and service delivery - with a focus on those groups that have been inadvertently affected by covid.
  5. Prevention and healthy lifestyles: ensuring people stay healthy throughout their lives - starting with preconception and birth to ensuring every child gets the best start in life, and targeting those with the greatest need to reduce inequalities, whilst maintaining an effective universal offer for everybody
  6. Maintaining the identity of Telford and Wrekin whilst supporting the system: in partnership with the Integrated Care System (ICS) designing and developing a decision making framework at place to enable priorities to be delivered.

The White Paper - what does it mean at 'place'

On 11 February 2021 the Department for Health and Social Care set up the new proposals to streamline and update the legal framework for health and care - “Integration and innovation: working together to improve health and social care for all”.  This paper takes into account the feedback from organisations as part of a consultation that closed on 31 December.

The timeframe for implementation of Integrated Care Systems (ICS) remains 2022. 

The White Paper is important to the work of the TWIPP as it emphasises the importance and role of place based working.  In particular:

  • The recognition that real change happens at place;
  • Facilitation of integration between health, local government and wider partners, including the voluntary sector is needed to improve the health and wellbeing of local people;
  • The commitment that existing local partnerships and democratic structures should be based on the local authority footprint; and
  • The expectation that ICSs will delegate functions to place-level partnership.

The TWIPP, alongside the Health and Wellbeing Board, is very well placed to support the delivery of the White Paper alongside the system wide ICS.  The updated TWIPP Strategic Plan already starts to address the areas mentioned within the White Paper and provides a solid foundation from which to build on over the next year.

Exciting Social Prescribing Work in Telford and Wrekin

In 2019 NHS England invested in developing a network of social prescribing link workers so we wanted to take this opportunity to update you on local arrangements and to share some of the excellent work happening in Telford and Wrekin.

Working across our Primary Care Networks we currently have 7 link workers providing support to patients that have been referred by their GP. Our dedicated team of link workers spend time with the patient and support them to co-produce a non-clinical social prescription to improve their health and wellbeing – a big part of this is signposting and helping people to access support in the community close to where they live.

In September 2020 we established a small steering group to develop our local approach. So far we have:

  • Established a link worker network
  • Secured a training grant of £5000 from Health Education England
  • Partnered with Energize Active Partnership and Shropshire Council to secure funding from National Charities Together to deliver a Green Social Prescribing Programme
  • Recruited a Community Sport & Health Apprentice within the Sutton Hill Partnership
  • Developed the social prescribing functionality of the Live Well Telford online directory
  • Secured funding to develop a Cultural Resource Hub

Over the next 6-12 months we plan to:

  • Expand the network to include more partners
  • Engage all link workers and community practitioners in a training programme to develop their knowledge and skills
  • Apply for grant funding to further develop our approach
  • Connect with our Active Partnership, Natural England and the Arts Council to develop our community offer
  • Map our social prescribing pathway to identify areas requiring more focus
  • Continue to develop partnerships with our community hubs
  • Recruit Community Connectors to support patients to connect with community support
  • Deliver a 2 year Green Social Prescribing Programme
  • Pathway development with wider support services (debt advice, housing)

Place based mental health – what’s been happening in the last few months?

Over the past few months there have been a lot of developments in what is on offer to support people with their mental health in Telford and Wrekin.  

Here are some of the highlights:

  • Calm Cafés, delivered in partnership between Telford Mind and Telford & Wrekin Council’s mental health team, has supported 196 people and Branches services received 113 new referrals in Quarter 3. This offer will be enhanced by additional short term services commissioned by Midlands Partnership Foundation Trust in order to support timely mental health discharges and preventing admission.
  • Discussions are ongoing around how local voluntary sector organisations could provide solutions to offer non clinical crisis interventions.
  • There has been a 47% increase in Mental Health Act Assessments since 2015/16 so strengthening the community and place based offer is an essential part of the local offer and developing a preventative approach.
  • The Rough Sleepers Task Force has developed its approach to multi agency and multi-disciplinary support with both mental health social workers and nurses from MPFT active members of the discussion, offering advice and guidance to those working with rough sleepers.

Consultation seeks views on five-year strategy for residents with learning disabilities

A new five-year strategy has been drafted to make sure all people with a learning disability in Telford and Wrekin live well & enjoy a full life, as independently as possible.

It’s based on feedback received at the end of last year from over 160 people - including those with learning disabilities and their families as well as carers, advocates and staff from various organisations.

Tell us what more can be done for all people with a learning disability in Telford and Wrekin to live well, as independently as possible.  Should borough residents with a learning disability have:

  • more employment opportunities?
  • more flexible support available?
  • easier & safer access to public transport?

What does good look like for older adults in Telford and Wrekin?

This Spring Telford and Wrekin Integrated Place Partnership are holding two virtual integration workshops to look at ways in which we can work together to improve outcomes for older adults in Telford and Wrekin.  The interactive sessions will include hearing people’s experiences, listening to your views and virtual integrated workshops.

By the end of the workshops we hope to:

  • Understand what the current offer is and where the overlaps are
  • Identify areas where we could work better together
  • Agree a key set of principles for how we work differently to improve outcomes, prevent duplication and increase capacity across the services

The workshops are suitable for:

  • Practitioners across health and social care organisations
  • Voluntary, Community and Social Enterprise sector workers
  • Care sector staff

An Independent Living Centre, that will help people with care and support needs to live well and as independently as possible is due to launch in the borough from 12 April

The centre is a joint venture between Telford & Wrekin Council and community partners,  including Telford & Wrekin CVS (Council for Voluntary Service), who are the joint leads on the project.

As well as a physical centre near Telford Shopping Centre, the project will also see an online virtual house on Live Well Telford - providing advice on aids and gadgets that both younger and older residents with care and support needs can use to live independently.

The physical centre will offer a broad range of support for people, including advice and the ability to try out aids and equipment to help them live independently. Appointments will available from 12 April.

An easy-to-navigate virtual house will go live online from mid-April and will be accessible to people 24/7. From the comfort of their own homes, they can learn about useful aids and gadgets that can be used within a family setting to help them or their loved ones continue to live as independently as possible.

How well is Urgent Medical Care working in Telford and Wrekin?

From December 2020, NHS England launched a new system called NHS 111 First. If you have an urgent, but not life-threatening health problem you can now contact NHS 111 First to find out if you need to go to A&E.

The public are encouraged to call NHS 111 to be directed to the correct service, whether that is an Emergency Department (Accident & Emergency), their GP, or self-care. Callers to NHS 111 can now receive time slots at local Emergency Departments (EDs) or other urgent care departments, as well as receive time slots with GPs or out of hours GP services. The new service aims to reduce waiting times at EDs and limit the number of people in waiting rooms, to prevent the spread of COVID-19.

Initial findings in national research are showing that many people do not know that NHS 111 First could book them into the different services they may need. Healthwatch Telford & Wrekin would like to hear from people who have used NHS 111 and how they were helped to access services.

Become a patient involvement advocate by joining the new CCG’s Assuring Involvement Committee

We’re inviting local people to join the new CCG’s Assuring Involvement Committee (AIC). This Committee of 12 community representatives advises us on the best ways to communicate with and engage local people. The AIC has an important role as a critical friend to challenge our communications and engagement work. 

The deadline for applying is 5pm Sunday 11 April 2021.

COVID-19 vaccination programme gathers pace in Telford and Wrekin

Half of the adult population in Telford and Wrekin has received their first vaccination dose, which is fantastic news.

People in early priority groups will be receiving their second vaccination soon.

Everyone who has had their vaccination still needs to follow the public health guidance and lockdown rules.

Adult Social Care Peer Challenge - Pause and Learn

Telford & Wrekin Council are part of the Association of Directors of Adult Social Services (ADASS) West Midlands sector-led improvement programme, part of which is the opportunity to do a Peer Challenge.  This is a key part of the Council’s approach to continual learning and development and the outcomes of the peer challenge will help to inform the direction of travel for Adult Social Care.

Due to the restrictions of Covid the Peer Challenge, which runs from 22 to 25 March, will take the form of pause and reflect informal conversations with the Challenge Team and a cross section of people and will be based on the key lines of enquiry (KLOEs).  This will include:

  • people with care and support needs,
  • their carers and families; and
  • A cross section of employees, providers and partners.

The Key Lines of Enquiry for this Peer Challenge are the impact of our partnership arrangements (ASC leading at Place) in Telford on local people with care and support needs and the Covid legacy impact on resources linking to market management.
