Return to Work Form
Dear Colleague,
Lead Employer are always looking at ways of improving our service. Following recent feedback from a number of our stakeholders, we have reviewed the Return-To-Work documentation and have now created an online form.
Our hope is by cutting out paper forms and accepting online form submissions that this will be less time-consuming for both supervisor and trainee.
Once the online form has been completed, Lead Employer will store this information securely on the trainee’s personal file.
