The Children with Disabilities Team has created a new Transition Plan that was co-produced with some children from Lancaster School.
This has been an 18-month project which has focused on the voice of the young person. The form has been rewritten with the comments and suggestions of the students but still captures what is needed to assess and inform the transition plans for children and young people towards adulthood.
The new Transition Plan was presented at our Improvement Board on 30 September 2024 with very positive feedback and with approval from senior management has now been added to our electronic system (LCS) that is accessible to all practitioners.
The Transition Plan is completed for all young people already in receipt of services from the Children with Disabilities Team and start at age 14 years. Over the next few months, the team will be developing easy-read formats and feedback forms for our young people and their families as well as reviewing their documentation to ensure that it continues to reflect the team/service provision and supports young people to achieve a smooth transition to adulthood.

The Elective Home Education Team's aim is to ensure that every child has access to the tools they need to succeed, even where education happens outside the usual school system.
We aim to provide families with similar access to key information that would normally be accessed at school. Our strong partnership working with the school nursing service, Connexions, SEND and SENDIASS teams are great examples of how we ensure all home educating parents and children have access to the advice and guidance they need.
We also host events like our GCSE Information Day to encourage EHE learners to sit formal exams. While home education allows flexibility, taking formal qualifications like GCSEs can be a challenge, as families need to self-fund exam entries and tutoring. Events like ours help bridge the gap, providing vital information and connections that empower parents and learners to make informed choices.
To find our more about Southend’s EHE offer and what’s on, visit our EHE Hub.
