Transition Support
Amended Education Health and Care (EHC) Plans have been issued this month and many young people with SEND will be preparing to transition to a new educational setting in September. The Local Authority has a range of services that can provide support to families throughout this transition period.
Information on the transition support provided by each service can now be found on their Local Offer pages.
Post-16 SEND Transition
For many young people with SEND, an educational transition is accompanied with a transition into young adulthood. This big change involves preparing for more independence and making plans for the future. For young adults with SEND, this transition needs more preparation and planning than for other young people of the same age.
A Preparation for Adulthood Guide has been created for young people, parents, carers and professionals to detail the education, health and social care services that can provide support throughout the different areas of life.

There are a wide range of events coming up in 2024 to support young people with their preparation for adulthood.

Sheffield Parent Carer Forum Peer Support Service
The Peer Support Service is available to all parents who would like advice, support and guidance especially during times of transition.
Staff can help parents navigate the wealth of information contained within the Transitions Guide making this as accessible as possible.
Make Your Mark Voting
Voting for Make Your Mark is open until the 8th of March 2024.
UK Youth Parliament's Make Your Mark helps form leaders of the future. It provides young people with a unique opportunity to let decision makers know the issues they're most affected by.
Voting is open to all young people aged 11-18 in the UK.

National Star has joined forces with fellow disability charity Together Trust to improve the way young people with special education needs and disabilities (SEND) are supported when they leave education.

Autism Pre-Diagnosis Support Survey
Rotherham Parent Carers Forum (RPCF) are currently seeking families experience around pre diagnosis support for autism.
They are keen to hear from all families and young people aged 5 - 18 across South Yorkshire who are currently awaiting diagnosis or who have been diagnosed within the last 12 months to find out what has been beneficial for them.

ADHD Drop In Café & Info Sessions
The Family Action ADHD Project offers Drop in Café mornings and Information sessions to all families affected by ADHD.
No booking is required for these sessions.

The Youth Voice and Influence Service have created a film with young residents in Sheffield who are LGBTQ+. They wanted to make something that was about acceptance and discovered in our conversations that acceptance was often found in small moments shared between family and friends.

Information day about anxiety, sensory challenges and behaviour
Friday 8 March, 10am-2.30pm, at Niagara Conference Centre
Sheffield Parent Carer Forum (SPCF) are running an information day for parents and carers of children and young people (0-25 years) who are experiencing anxiety, sensory processing difficulties and/or behaviours that challenge.

Sheffield Children’s Information Resource Library has lots of health information for you to look at and read. There is advice on autism, speech and language, epilepsy, hearing and sleep to name but a few.

Parents Information Sessions
Sheffield SENDIAS (Sheffield SEND Information and Advice Service) offer free online information sessions and webinars. These are for parents and carers of young people aged 0-25 with SEND. The sessions answer some of the most frequently asked questions.
