Two posts available

To contribute to ensuring that the physical, emotional and identified primary care needs of children are met through the development and maintenance of a supportive and caring environment.
To contribute to establishing positive relationships with the children, assessing needs and working jointly with other agencies in preparation, implementation, delivery and revision of care plans/Pathway Plans to improve outcomes.
To communicate professionally and effectively with children and families, other team members, professionals from statutory or voluntary sectors and other agencies.  
To work in line with relevant legislation and policies pertinent to the job.

ESSENTIAL REQUIREMENTS:

1. An understanding of the Social, Cultural, physical and emotional needs of children, especially those with challenging behaviour
2. An ability to communicate effectively with young people, families, other agencies and the team
3,Ability to contribute to the assessment, planning, implementation and monitoring of individual care programmes for children in key-working or co-working capacity to improve their outcomes
4, An ability to work in a flexible manner in a sometimes challenging environment
