If you are a qualified social worker with experience of successfully managing activity and performance of individual staff then we have an exciting opportunity for you.

The Hospital Rapid Recovery Team follows service users who are discharged from hospital into residential placement with the aim of supporting the service user to regain independence and be supported to return home. The worker will be expected to engage with a strength based approach and be proactive in supporting people to have their needs met in their own home. People are often discharged from hospital but are not back to their baseline and need further time to recover, this means that people’s needs change significantly after discharge, the worker will monitor and assess progress within the residential setting and complete a final assessment to see where best the needs of the service user can be met.  

You will work as part of an integrated multi-disciplinary team to provide high quality social work practice, demonstrating expert and effective practice in complex situations, assessing and managing higher levels of risk, striking a balance between support and control, and liaising with a wide range of professionals.

Please note that 7 day working including bank holidays will be required and it is  anticipated this will not be more than 1 weekend in 3.  Further details will be discussed with you at interview.
