There has been significant investment in additional posts to support the transformation and Hull City Council are looking for exceptional individuals like you to join the Adult Social Care team.

We are seeking people who have the energy, motivation and innovation to pursue a career within adult social care. This is an exciting opportunity to make a real difference in embedding the principles for personalisation, and delivering key aims of early help and prevention to work with people who may require support in achieving personal aspirations and goals relating to maximising their independence.  The role is implicit in the development and delivery of imaginative and creative solutions which underpin the requirements of the Care Act 2014 and, essentially support Hull’s strategic vision of “a life not a service”.

We are looking to offer exciting opportunities for people who can demonstrate their ability to focus on people’s strengths and choices to live their lives to the full by working in partnership with people their families and their communities.

The successful candidate will have the following training, experience and knowledge:

• BSC in Occupational Therapy  or equivalent qualification
• Substantial post-qualification experience of working with adults in a statutory health or social care setting
• Be registered with the HCPC and able to evidence this
• Experience of safeguarding legislation including the Mental Capacity Act and Deprivation of Liberty safeguards.
• Evidence of continuing professional development
• Experience of coaching, mentoring and supervising other staff
• To undertake specialist OT assessments to determine eligibility, mental capacity, physical capabilities and ongoing needs including for example Independent Mobility Assessments (Blue Badge) and re-housing. 

The role requires you to contribute to specialist or comprehensive assessments for disabled people and their carers to recommend a range of help available.

Due to the recent restructure and internal promotions, there are a number of posts, both full time and part time, available within the Locality teams and Active Recovery teams.

 

Please note that 7 day working including bank holidays will be required for Hospital and Active Recovery and it is anticipated this will not be more than 1 weekend in 3.

Active Recovery Assessment and Rapid Recovery

Active Recovery is used to proactively work with people over a defined period of time to achieve goals with the overall aim of maximising their independence, health and quality of life. It is aimed at adults who have a new or longer term need for services but could be supported to recover or minimise their needs, reducing reliance on longer term care. Active Recovery operates differently from traditional home care as it focuses on helping people regain the ability to do as much for themselves as possible rather than doing things for them –demonstrating how to achieve something and then gradually standing back to encourage people to develop these skills

Rapid Recovery is working with people placed into step down/respite beds to support them home as quickly as possible . You will work in a person centred, solution focused way and assessments and reviews will focus on identifying and updating people’s individual goals and their ability to achieve them.

 

East & West Locality Teams

The East and West Locality teams work in partnership with all individuals requiring longer term adult social care support. Referrals to the teams will come directly from See & Solve Community, for ongoing support following Active Recovery and after a hospital discharge package has been put in place. The teams work jointly, between Social Work, Occupational Therapy as well as with Sensory and Telecare colleagues, to identify actions and goals the individual wants to achieve and the best way to achieve these and are within statutory frameworks including the MCA 2005 and Care Act 2014. The teams also ensure individuals receive timely and proportionate reviews ensuring that an individual’s goals and actions are reflecting changes in their life and supporting them to be independent.
