Are you passionate about making a positive and lasting difference for the people of Hull?  Are you able to contribute to achieving our vision of enabling people to have a life not a service?

The transformation programme we have embarked upon and making a success of our new operating model is key to ensuring the work we do is sustainable and effective in the future. There has been significant investment in additional posts to support the transformation and Hull City Council are looking for exceptional individuals like you to join the Adult Social Care team.

You will work as part of an integrated multi-disciplinary team to provide high quality social work practice, ensuring individuals have a solution focussed plan towards achieving their goals and maintaining their independence.  You will work creatively with individuals, local communities and our partners, empowering them to deliver the best possible outcomes for people and to create a positive Adult Social Care culture.

We are looking for individuals who are qualified and experienced social workers who can demonstrate:

  • a strong value base that informs your work with adults
  • a relentless commitment to quality - and to be able to talk about what good practice 'looks like'
  • a capacity for honest reflection and openness to learning;
  • ability to establish professional, effective working relationships with a range of partners/colleagues, adults, families and their carers

All Social Workers must be able to provide and support the delivery of high quality social work activities however, we have opportunities for different levels of experience as follows:

Grade 8: £28,221 - £30,756

This post is for you if you have completed your ASYE and can demonstrate increased decision making autonomy in situations of increasing complexity, risk, uncertainty and challenge.

Grade 9: £31,401 - £34,106

This post will provide and support expert and effective social work practice in complex situations, assessing and managing high levels of risk and providing expert opinion in relation to decisions surrounding complex cases. You will undertake a professional leadership role taking responsibility for the learning and development of others, through leadership and management, supervision, mentoring, assessing and teaching.

Due to the recent restructure and internal promotions, there are a number of posts available within Locality, Hospital and Active Recovery teams.

Please note that 7 day working including bank holidays will be required for Hospital and Active Recovery and it is anticipated this will not be more than 1 weekend in 3.

Your preferences of team and rota will be discussed with you at interview stage. Details of the teams are outlined below:

Active Recovery Assessment and Rapid Recovery

Active Recovery is used to proactively work with people over a defined period of time to achieve goals with the overall aim of maximising their independence, health and quality of life. It is aimed at adults who have a new or longer term need for services but could be supported to recover or minimise their needs, reducing reliance on longer term care. Active Recovery operates differently from traditional home care as it focuses on helping people regain the ability to do as much for themselves as possible rather than doing things for them –demonstrating how to achieve something and then gradually standing back to encourage people to develop these skills

Rapid Recovery is working with people placed into step down/respite beds to support them home as quickly as possible. You will work in a person centred, solution focused way and assessments and reviews will focus on identifying and updating people’s individual goals and their ability to achieve them.

Hospital See and Solve & Hospital Locality

The Hospital See & Solve Team supports a person being discharged from hospital and the team play an important role in ensuring that people’s independence is maximised and that those people who formally enter the system do not unnecessarily grow dependent on it but strive to maximise their strengths and achieve their goals. The teams use a solution focussed approach to discuss what is important to people, and use their expertise and knowledge of the community to help them identify options and commit to action.

The Hospital Localityteam supports all individuals requiring longer term adult social care input on discharge from hospital; working with health professionals to assess their immediate needs, identifying the goals they would like to achieve and the support required to meet these, working with Brokerage to identify suitable providers; ensuring a timely and coordinated discharge from hospital.

East & West Locality Teams

The East and West Locality teams work in partnership with all individuals requiring longer term adult social care support. Referrals to the teams will come directly from See & Solve Community, for ongoing support following Active Recovery and after a hospital discharge package has been put in place. The teams are work jointly, between Social Work, Occupational Therapy as well as with Sensory and Telecare colleagues, to identify actions and goals the individual wants to achieve and the best way to achieve these and are within statutory frameworks including the MCA 2005 and Care Act 2014. The teams also ensure individuals receive timely and proportionate reviews ensuring that an individual’s goals and actions are reflecting changes in their life and supporting them to be independent.
