A single health and care integration programme for East Sussex

East Sussex views

Over the last few years the East Sussex Better Together (ESBT) Alliance partners have been working closely together to improve population health and wellbeing and reduce health inequalities, by breaking down barriers between health and social care, to deliver the right services, in the right places, at the right time. In High Weald, Lewes and the Havens, similar work has been taking place through the Connecting 4 You (C4Y) programme.

To build on these successes and strengthen our work across the county, we now plan to bring the two East Sussex programmes (ESBT and C4Y) together, to establish a single health and care programme across the whole of East Sussex.

By working in such a joined-up way, we believe we’re able to serve the whole of East Sussex even more effectively.

Take part in the survey to help parents and carers support young people's emotional wellbeing

Royal visit to Joff Youth Centre to discuss young people's mental health

East Sussex Youth Cabinet and Seaford Youth Forum are working with Seaford Head school, supported by East Sussex County Council and the Clinical Commissioning Groups, to create an information campaign for parents and carers to help them better support their children’s emotional wellbeing.

To help shape the campaign you can take part in a survey to give your views.

Mental Health and Wellbeing Prescription Pad

Child and parent playing together

In July 2018 a new initiative called the ‘The Mental Health and Wellbeing Prescription Pad’ launched in GP surgeries in Hastings and Rother, and from April 2019 it will be rolled out to more GP practices in East Sussex.

Part of our East Sussex commitment to ‘Think Family’, the Prescription Pad helps GPs to signpost parents and young people with a range of emerging emotional wellbeing concerns to projects that offer early help, without the need for a formal referral.

Maximising independence with Extra Care

Extra Care housing

You’ve probably heard of residential care and sheltered housing, but have you heard of Extra Care?

Extra Care housing is for older people with care and support needs who want to continue to live independently. In East Sussex we have seven Extra Care housing schemes, which offer a range of facilities to residents.

Read the full article to learn more about how Extra Care housing supports its residents, and who can access it.
