The Health and Wellbeing Team are the Community Builders for Social Prescribing. We work in partnership with the NHS Community Links, The British Red Cross and The HEY Smile Foundation.
Our team's role is to find, support and create social and activity groups for people to attend to improve their health and wellbeing.
