‘One Devon’ Hospital discharge booklet

‘Planning Together and Leaving Hospital: Returning Home or to another place of Care’ is a booklet containing lots of useful advice and information about hospital discharge in Devon. Published on behalf of the ‘One Devon’ health and care system, this 9-page publication applies to people leaving hospital and returning to all settings, not just to care homes.

Extra Care Housing Market Engagement Opportunity Webinar Next Week

Devon County Council (DCC) is seeking to establish a Flexible Purchasing System (FPS) for Extra Care providers able to deliver care and support services into Extra Care Housing (ECH) schemes in Devon.

The FPS will be comprised of two Lots:

Lot 1 - Care and Support Services:

  • Core provision to ensure care and support cover 24-hours a day including social inclusion and response to unplanned care needs
  • The provision of commissioned one-to-one care and support that a significant number of people living in Extra Care Housing require and for which they are eligible under the Fair Access to Care Assessment criteria.

Lot 2 - Innovation and Future Working

  • This Lot will enable DCC to continue to develop its ECH model over the lifetime of the FPS, giving providers opportunities to innovate and work in different ways when opportunities arise.

DCC currently commissions care and support into six Extra Care Housing schemes, of which four schemes will be called-off for immediately, as part of the tender to establish the FPS.

An additional two services are currently in development with planned completion dates between Autumn 2027 and Spring 2028, and further schemes are at the planning stage and anticipated to be completed within the lifetime of the FPS.

Contract opportunity: Pathway 2 Block Booked Beds in Eastern Devon

NHS Devon Integrated Care Board is intending to commission 10 Pathway 2 Block Booked Beds for a 12 month contract in Eastern Devon at a cost of £1,000 per bed per week. The provider given the highest score will be able to provide 10 beds, or be willing and able to offer additional beds (to a maximum of 10) as vacancies become available. If there are not any providers who can provide a total of 10 beds, then consideration maybe given to awarding a contract to two different providers.

Pathway 2 provision is defined as: Likely to be maximum of 4% of people discharged from the acute hospital: the main aim is to provide recovery, rehabilitation, assessment, care planning or short-term intensive support in a 24-hour bed-based setting to maximise levels of independence, before the person is supported to return home.

Only Providers who are CQC Registered Care Home Provider (65+) with a CQC rating of 'Good' or above who can commit to providing block booked beds at the above price per bed per week will be considered.

Devon County Council Integrated Adult Social Care Commissioning Team Restructure Update

Devon County Council is currently re-organising its Integrated Adult Social Care Commissioning team, which is being managed in two phases.

Phase 1 of the restructure focused on grades below senior leadership level with a realignment of functions broadly into two areas:

  • the merging of the strategic element of Market Management, Workforce and Commissioning function into one Strategic Commissioning and Market Development Team
  • the merging of the tactical element of Market Management team functions of contract management and quality assurance and improvement creating the Quality and Contract Management team

Phase 2 focused on senior leadership grades with the aim of establishing a structure aligned to and proportionate with the new smaller team, whilst ensuring that the wider team still has the capacity and skills to deliver the ongoing and significant savings, as well as our market sufficiency duties.  That has meant removing three senior leadership roles and bringing in more capacity at lower leadership grades to deliver those essential functions.

Forms of  ID which can be used when Voting 

When people go to vote, they will need to have photo ID. We know that people with learning disabilities may not have a provisional driving license or a passport, but there are alternatives that are more readily available:

A Disabled Persons Bus Pass – often the photo bus passes are accepted as voter ID. The caveat to that is it must be ‘government funded’.

Blue Badges – because these are issued by a local authority, Blue Badges are accepted as forms of ID.

A Voter Authority Certificate – these are free forms of ID that require you to be registered to vote before applying. If you are registered to vote, then you can apply online for the Voter Authority Certificate.

The deadline to apply for a Voter Authority Certificate to vote in the General Election is next week: Wednesday 26 June.

Good News Story: Care Home staff Alzheimer’s Society fundraising walk 

Care workers and managers from Hill House in Torbay and Three Corners in Brixham recently took part in a 13 mile walk to raise money for the Alzheimer’s Society.

Congratulations to Marta Broyd, Michelle O’Sullivan, Carol Ashmore, Charley Hambly, Lisa Clark and Natalie Oleski.

NHS and Care Volunteer Responders

The Volunteer Responders programme – now named NHS and Care Volunteer Responders - is being expanded to support adult social care providers in England. You can now arrange volunteer support for recipients of social care, including domiciliary care and people living in care homes. The volunteer services on offer include:

  • Check in and Chat – short-term telephone support consisting of friendly conversations to improve mental health and wellbeing.
  • Pick up and deliver – transportation of medication to people’s homes from NHS sites.
  • Community Response (coming soon) – help with activities including collecting and delivering food shopping and essential items and / or prescriptions and medications.

The apprenticeship levy is open, £26,000 levy funds available for social care. Devon County Council can transfer apprenticeship levy funds to other employers in Devon. You can use this fund for apprenticeships for your staff to help develop your workforce. This includes new apprentices or existing employees. Find out more.

Made-Well Transitions Open Day on 2 July

Made-Well at Hatherleigh are holding a Transitions Open Day to showcase the support they offer for young people with a learning or physical disability, mental health issues and/or other related needs.

  • You may be leaving school / college unsure what to do next.
  • You may not be attending school or only attending core subjects.
  • Are you a social worker interested in finding out more about local opportunities for people in the area.
  • Maybe you have a family member who would benefit from getting out and about, making friends and connections.
  • Perhaps you are an organisation/provider or a care home supporting young people and adults, who would benefit from meeting others and socialising.

The cafe, inclusive playground and allotment garden will all be open. There is a Changing Places toilet and a sensory room available for hire and the site is wheelchair accessible.
