Digitalising Social Care 10 minute survey - reminder!

As part of the National Digitising Social Care (DiSC) Programme the One Devon (Devon, Plymouth, Torbay local authorities and NHS) project team, are looking for all CQC registered Providers to complete a 10-minute One Devon survey. The survey results will enable us all to understand the current market position and how, with potential match funding, a plan could be coproduced to work together to deliver the DiSC programme in Devon.

A summary of the feedback will be shared with you, so as well as supporting the development of the Devon ICS plan, all providers will benefit from understanding the barriers to adoption, and experience by those who have already introduced digital technologies, example case study here.

This is not a one-off project, but the start of a period of co-creation. Please take part if you can.

More information can be found here.

Royal Devon Hospital Treatment Escalation Plans (TEP)

Royal Devon University Healthcare NHS Foundation Trusts' new electronic patient record system (Epic) can generate patient Treatment Escalation Plans (TEP) for patients discharged from hospital. These paper documents may visually format slightly different to what care providers are used to seeing.

However, it is important to state is these TEP's are valid documents (despite looking slightly different) and all the required information is still captured within the document.

If after discharge the TEP is then updated by the GP it is this updated version which staff should follow as this supersedes the TEP sent home with the patient from hospital.

If you have any questions, please email Rduh.epiccommunity@nhs.net
