Call for help with hospital discharge pressure

Residential and Nursing Care Homes only

East Devon’s system partners are currently experiencing unprecedented pressures in sourcing Nursing and Residential care home placements for people who are ready to be discharged from Royal Devon & Exeter Hospitals.

We are calling on all homes who have Residential or Nursing capacity available to:

ensure that their bed vacancy detail is recorded and up to date on the National Capacity Tracker.
where homes are approached by the Arranging Support Team/Bed Bureau to consider a referral that they offer all assistance, where possible, to support Hospital discharge arrangements and timely transfers.

When the Assessment of patients in Hospital can be supported remotely where appropriate, please utilise as this would expediate transfer arrangements
