Reports were sent via Perspective Lite General folder to schools. Please ensure you are using the new guidance dated 28 September 2021 to complete the forms. The deadline to submit has passed but any schools that have yet to respond are encouraged to do so as soon as possible.
Public Health would appreciate your assistance in completing a questionnaire to help them in developing a training programme to tailor training and resources for infection prevention and control measures to best meet your school requirements.
The deadline for submissions is 31 March 2022. Please ensure you use the revised SFVS submission document from the GOV.UK website and return completed documentation to Suzanne.kiernan@derbyshire.gov.uk, Perspective Lite (830AUD_).

Primary and infant schools can now see their provisional numbers for the 2022-2023 intakes on SAM. Please check numbers regularly as outstanding requests from neighbouring authorities and late applications are added. Please note the information regarding offering additional places and respond (if applicable for your school) back to: admissions.transport@derbyshire.gov.uk by Friday 11 February.
